眼科住院医师规范化培训中人文素养培育的实践与探索

Practice and exploration of cultivating humanistic qualities in standardized training of ophthalmology residents

在眼科住院医师规范化培训中,医学人文素质的培养对医患沟通能力与职业素养的塑造具有重要价值。医学人文不仅关乎医生的执业能力,更直接影响医疗服务的质量与患者满意度。文章分析了当前眼科住培医师人文素养教育现状及其重要性。在此基础上,提出了培养住培医师的人文素质的方法和途径:1)通过入培前的人文素养专题培训,强化住培医师的医患沟通意识,使其在进入临床实践前就树立正确的人文理念;2)借助医院服务中心的实践活动,让住培医师亲身参与患者服务,从而培养他们的同理心,更好地理解患者的需求与感受;3)开展医学人文读书活动,引导住培医师阅读经典人文著作,深化他们的人文情怀,提升职业素养;4)组织科室人文析议会,针对临床实践中常见的伦理困境进行研讨,帮助住培医师学会在复杂情境中做出合理决策;5)实施思政与医学人文融合的实践,拓展住培医师的人文视野,使其在专业技能提升的同时,不忘人文初心。该文旨在为眼科住培医师的人文素质培养提供理论支持和实践指导。通过这些方法的实施,期望培养出更多具备高人文素养的眼科医疗专业人才。
Strengthening medical humanities education is of vital significance in the standardized training of ophthalmology residents, as it contributes to enhancing their doctor-patient communication skills and professional ethics. This paper examines the current situation and significance of humanities education for ophthalmology residents. On this basis, the paper suggests five concrete methods and approaches for fostering residents' humanistic qualities, such as: (1) strengthening doctor-patient communication awareness through pre-training humanities courses to establish a proper humanistic mindset before clinical practice; (2) developing empathy by participating in hospital service center activities through hands-on patient service experiences; (3) deepening humanistic sentiment and enhancing professional ethics through medical humanities reading activities featuring classical works; (4) discussing clinical ethical dilemmas via departmental humanities analysis meetings to help residents develop decision-making skills in complex scenarios; (5) expanding humanistic horizons through the integration of ideological and political education with medical humanities practice, ensuring professional skill development aligns with humanistic principles. This paper aims to offer theoretical support and practical guidance for the humanities education of ophthalmology residents, with the objective of cultivating more ophthalmology professionals with both clinical excellence and high humanistic caliber.

        眼睛不仅是视觉器官,更是心灵的窗户。研究表明,视觉障碍除导致焦虑抑郁外,还显著影响患者的收入、就业及生存质量[1]。以青光眼患者为例,他们常表现出焦虑和强烈的控制欲,这要求医生必须展现出耐心和同理心,从而建立信任并提供个性化医疗服务[2]。一项针对白内障手术患者的研究发现,通过术前详细解释手术流程和术后注意事项,患者的术后满意度提高了24%,有效地沟通能减少患者的术前焦虑水平,使其对康复过程更加积极[3]。因此,眼科医生除需具备专业知识外,更需要具备敏锐的心理洞察力[4]

        现代医学提倡生物心理社会模式,要求医生在治疗疾病的同时关注患者的心理和社会背景[6]这一点在眼科尤为重要,因为眼科患者中老年人占据相当比例,他们往往更加渴望得到关注和理解,这要求医生在提供医疗服务时展现出对这一特殊群体的细致关怀[8]。高水平的眼科学教育应注重人文素质培养,从而显著提升医生的沟通技巧和同理心,促使医生更好地理解患者的需求,增强医患关系的质量[9]

当前,住院医师规范化培训是我国医疗体系的重要组成部分,住培医师完成培训后,就承担起了各层级临床医疗工作,与我们国家未来临床专业的水平休戚相关。然而,现有的培训体系中人文素质培养仍略显薄弱,人文素质培养相对专业技能训练的比重仍然相对较低[10–12]这种情况与当代医疗服务的全面需求不相匹配[13–15]。因此,本文总结了笔者多年在来眼科住院医师规范化培训阶段人文素养教育的有效策略,以期为培养具备专业技能与人文素养的眼科医师提供理论依据和实践参考。

1  眼科住培医师人文素养培育的实践与探索

1.1 入培前人文素养专题培训

在住培医师正式进入眼科住院医师规范化培训基地之前,医院及科室两级联合组织了人文素养和医患沟通的专题培训。培训通过讲座和工作坊的形式,重点强调了医患沟通的重要性和技巧,以及如何在临床实践中体现人文关怀。资深医师和人文专家通过分享实际案例,引导住培医师深入讨论和反思,增强了他们对患者情感和需求的理解和同理心。角色扮演和小组讨论的互动环节,使住培医师有机会实践沟通技巧,提高了他们解决潜在冲突的能力。培训结束后,住培医师普遍反映在沟通技巧和人文关怀意识方面有所提升,为即将到来的临床工作打下良好的基础。

1.2 参与医院服务中心实践活动

       我们创新性地将住院医师培训与医院服务中心工作相结合,安排一年级住培医师参与患者满意度调查与医疗投诉处理等实践活动。在满意度调查过程中,住院医师通过直接与患者互动,深入了解患者需求,提升沟通技巧和同理心。在医疗投诉处理环节,住院医师能够实地观摩和学习如何妥善处理复杂医患关系,使用合适的语言和肢体动作等,安抚患者情绪,妥善处理医患矛盾。并在实践活动后进行经验总结和心得分享,促进住培医师深化对医者仁心的理解,增强社会责任感和使命感。

1.3 融入医学人文的读书活动

       为了进一步增强住培医师的人文情怀和素质,我们不定期组织住培医师参加医院设立的“惠世书享汇”人文读书计划,采用“导读-分享-讨论”的模式进行。主要研读《医学人文十五讲》等医学人文领域的经典书籍。通过读书分享和小组讨论,住院医师逐渐培养出更强的同理心和人文情怀。这种模式不仅促进了知识的共享,还激发了深层次的思考和自我反省[16]。住培医师在讨论中积极表达自己的观点,同时也倾听并吸收同伴的见解,这种互动丰富了他们的人文视野,有效提升了他们应对压力和职业倦怠的能力。许多参与者表示,通过阅读和讨论,他们对医患关系有了更深入的理解,在临床工作中也更善于倾听患者的需求。

1.4 按需组织科室人文析议会

       为提升住院医师的人文素养,我们科室定期举办人文析议会,探讨临床工作中医患沟通问题和人文相关的大事小事。譬如,白内障摘除后进行人工晶体植入是眼科最常见的手术。科内不少医师习惯将单焦点人工晶体称为“普通晶体”,而将多焦点人工晶体称为“高端晶体”。然而,在一次人文析议会上,有住培医师提出,这种区分可能会无意中传达出一种价值判断,即多焦点晶体比单焦点晶体更优越,从而可能让患者感到自己接受的治疗不够先进或不够好。这种称呼还可能引发患者之间的比较和竞争心态,导致不必要的焦虑和压力。因此,建议我们改变这种称呼习惯,采用更为中性和包容的术语,将人工晶体称之为“单焦点晶体”和“多焦点晶体”,或者根据晶体的具体特性来命名,如“非球面晶体”和“功能晶体”。这样的命名不仅避免了对患者造成不必要的心理负担,也更准确地反映了晶体的技术特点,有助于患者理解他们接受的治疗方案。通过这种小小的语言调整,我们能够更好地尊重每一位患者,无论他们的选择如何,都能感到被平等对待和尊重。这不仅有助于建立医患之间的信任,也体现了我们作为医疗专业人员的人文关怀和专业素养。

1.5 思政医学人文融合实践

       为拓展住培医师的人文视野和政治素养,我院适时邀请党校教师开展医学人文专题讲座和思政教育。邀请党校教师深入讲解政策理念,强化医师的法律意识和责任。同时,与心理咨询中心联合,为住培医师提供专业心理辅导,帮助他们在高压环境下保持良好的心态。此外,我们还定期组织跨专科的交流会,分享不同领域的人文关怀经验,促进知识的综合与思维的拓展。通过案例分析和角色扮演等互动教学,使住培医师在实践中深刻体会思政、法律与医学人文的融合,从而在复杂医疗情境中作出更全面的判断,实现素养的全面提升[17–19]

通过以上措施的系统实施,我们观察到住培医师在医患沟通、医疗纠纷处理等方面都有明显提升。临床带教老师反映,参与培训的住培医师在面对复杂医患关系时表现得更加成熟,展现出更强的职业素养和人文关怀。这些观察表明,有计划、有步骤的人文素质培养对提升住培医师的综合能力具有重要作用。

2 眼科住培医师人文素养培育的成效评估

2.1 评价体系的构建与实施

       科学合理的评价体系是培养住培医师人文素质的关键我们构建了包含以下维度的综合评估体系:1)医患沟通能力评估,通过标准化患者访谈和临床实践观察进行;2)同理心水平测评,采用医生共情量表进行定期评估;3)职业素养考核,结合临床带教老师评价和科室360度评估;4)患者满意度调查,通过问卷和访谈收集反馈。将上述评估内容进行量化处理并切实落实到位,不仅能够科学、有效地评估眼科住院医师人文素养培育的成效,更有利于住院医师和带教老师清晰地认识到自身的优势与不足,从而更好地开展自我评价与自我提升,推动住院医师人文素质的持续进步。

2.2 培育成效的综合分析

2.2.1 个人发展与专业成长

通过以上人文素养培育的实践与探索,住培医师在专业知识和人文素质上取得了显著进步,尤其是在沟通技巧和医患关系处理方面,表现得愈加成熟、自信,综合素质明显提高。同时,参与社会服务与跨学科合作显著增强了他们的社会责任感和职业素养,许多结业生在医学人文关怀领域持续深耕,成为优秀的医务工作者,树立了良好的职业形象。这一系列的提升为他们的职业生涯打下了坚实的基础。

2.2.2 社会效益与患者反馈

通过对住培医师进行针对性的医学人文培训,本科室患者的满意度有显著提高,2023年以来,我院眼科门诊和病房医疗实现零投诉。这些培养措施对住培医师的职业发展也产生了积极影响,业后他们在各自的工作单位均表现出色,得到同行和社会的好评。

2.3 挑战与改进方向

尽管取得了显著成效,实践中仍面临挑战,如课程的全面性与专业深度的平衡、住培医师个体差异及实践资源和时间的限制。我们将优化课程设计,增强多样性和灵活性,强化个性化培训,探索更多社会实践平台。通过一系列实践探索,我们在眼科医师的人文素质培养上取得了显著成果。但同时,也需要不断总结经验、应对挑战,持续完善,为培养更具人文关怀的医学人才而不懈努力。

3 总结与展望

       文章通过分析当前眼科住院医师规范化培训中人文素养教育的现状,探讨了人文素质在眼科住培医师培养中的重要性,提出了一系列实施策略,并已在实践中取得初步成效。展望未来,眼科住培医师的人文素质培养将进一步优化培训体系,设立跨学科委员会整合人文教育元素,定期更新培训内容以适应时代发展;创新教学模式,融合线上线下教学,利用虚拟现实(virtual reality, VR)与增强现实(augmented reality, AR)技术增强实践体验[23–24],并结合人工智能(artificialintelligence, AI)重构教学模式,鼓励创新实践[25]。同时,加强师资队伍建设,通过定期培训和交流提升教师的人文教学能力,鼓励教师参与相关研究项目,以促进教学经验的分享。进一步完善评价与激励机制,建立科学的人文素质评体系,增加实操测评,并设立奖励以表彰在学习和实践中表现突出的住培医师,旨在培养更多具备综合素质的高水平医学人才,满足医疗行业需求,提升医疗服务质量。
