改良包眼方法对斜视双眼矫正术后患者视功能相关生活质量及焦虑情绪影响的非随机对照试验

An improved eye-covering technique: its impact on postoperative vision-related quality of life and anxiety in patients undergoing bilateral strabismus surgery-a non-randomized controlled trial

目的:探索采用一眼眼罩联合另一眼眼垫的改良包眼方式对斜视患者双眼矫正术后恢复期生活质量及焦虑情绪的影响。 方法:于2024年9—10月在中山大学中山眼科中心招募日间手术接受双眼斜视矫正术的患者80例,按入院时间单双周分为联合眼罩组和眼垫组,每组各40例。联合眼罩组在手术肌肉少的眼睛或主导眼佩戴多小孔塑料眼罩,另一眼用眼垫包眼;眼垫组采用双眼眼垫包眼。主要结局指标为术后1 d的视功能相关生活质量。次要结局指标包括术后1 d的睡眠评分、疼痛评分、球结膜充血水肿、切口外观及术后1周患者满意度。 结果:两组患者在性别、年龄、术前斜视情况和斜视手术肌肉条数等基线特征分布的差异无统计学意义(P>0.05)。联合眼罩组术后1 d视功能相关生活质量、睡眠和术后1周患者满意度均优于眼垫组,差异具有统计学意义(P<0.05)。两组患者在术后1 d疼痛程度、球结膜充血、切口外观、斜视改善程度的比较,差异无统计学意义(P>0.05);结论:斜视双眼矫正术后采用一眼眼罩联合一眼眼垫遮盖的方式,在保证术后恢复效果的同时,能够有效改善患者术后视功能相关生活质量和睡眠质量,并提升患者满意度。
Objective:To investigate the effects of a modified eye-patching technique, which combines the use of an eye shield on one eye and an eye patch on the other, on the quality of life and anxiety levels of strabismus patients during the recovery period after binocular correction surgery. Methods: A total of 80 patients who underwent bilateral strabismus correction as day surgery at our hospital between September and October 2024 were recruited for this study. Based on whether their admission week was odd or even, they were randomly assigned to two groups: the combined patching group (n=40) and the conventional patching group (n=40). In the combined patching group, patients wore a multiple-aperture plastic eye shield on the eye with fewer operated muscles or the dominant eye, while the other eye was covered with a conventional eye patch. In the conventional patching group, patients received bilateral eye patches. The primary outcome measure was vision-related quality of life on the first postoperative day. Secondary outcomes included the sleep quality score, pain score, conjunctival hyperemia and edema, incision appearance on the first postoperative day, and patient satisfaction one week after surgery. Results: There were no statistically significant differences in baseline characteristics, such as gender, age, preoperative strabismus status, or the number of operated muscles, between the two groups. Compared to the conventional patching group showed significantly better vision-related quality of life and sleep quality on the first postoperative day, as well as higher patient satisfaction one week after surgery (P< 0.05). On the first postoperative day, there were no significant differences between the two groups in pain level, conjunctival hyperemia, incision appearance, or degree of strabismus improvement (P > 0.05). Conclusions: The application of a modified eye-covering method, which combines an eye shield on one eye and an eye patch on the other, after bilateral strabismus correction surgery, can effectively enhance postoperative vision-related quality of life and sleep quality. Moreover, it can maintain recovery outcomes and increase patient satisfaction. This approach holds clinical value and is worthy of wider promotion.

        斜视是一种双眼视线方向不一致、无法同时注视同一目标的眼位异常疾病,它不仅影响外观,更重要的是会损害双眼视觉功能,尤其是立体视觉,在儿童时期还可能导致弱视,因此需要及早发现和治疗[1,2]。斜视作为眼科常见疾病之一,手术是其主要的治疗手段[3,4],在接受斜视矫正术的过程中,住院经历、手术创伤、双眼术后被完全遮蔽等因素均可能引起患者的消极情绪,从而影响患者的治疗效果和就医体验[5,6]。随着目前日间手术在我国的推广与不断完善,尤其是在眼科择期手术中的应用[7~8],斜视手术多实施日间手术,而传统的斜视双眼手术术后均用眼包遮眼,容易引发患者恐惧、行动不便和空间定向障碍,不利于日间手术患者往返医院复查和术后康复[9]。本研究对双眼术后包眼进行改良,创新性地提出一眼改用无菌且软硬适中的多小孔塑料眼罩遮盖术眼的术后遮眼方案,对比两种遮眼方式的临床效果,旨在探索更优的术后护理方案。

1 资料与方法

1.1 研究对象

        本研究是一项非随机对照研究,于2024年9—10月在本院招募接受双眼斜视矫正术患者80例,按入院时间单双周分为联合眼罩组和眼垫组,每组各40例。斜视的诊断标准参照1999年麦光焕教授主编的《现代斜视治疗学》[10]和2021版颜建华教授主编的《斜视临床诊疗指南》[11]。纳入标准:拟在我院日间病房行双眼斜视矫正术的患者;能够准确表达自身感受。排除标准:存在认知障碍及精神异常、合并有全身严重疾病、本院行二次斜视矫正手术,裸眼视力低于0.1者。本研究经过本院伦理委员会批准(批件号:2024KYPJ050),参与研究的所有患者或患儿家属均签署纸质知情同意书,自愿参加本研究。

1.2 包眼方法

        眼垫组患者在接受双眼斜视矫正术后,双眼均用5.5 cm × 7.5 cm医用吸水垫(眼垫)遮盖(图1)。联合眼罩组患者在双眼斜视矫正术后,手术肌肉较少的眼睛或同等肌肉量的主导眼使用经过消毒灭菌、软硬适中的橙色多小孔(57个直径2.5 mm小孔)倒三角弧形5.5 cm × 7 cm塑料眼罩遮眼,另一眼使用眼垫遮盖(图2)。

1.3 观察指标及测量方法

    1.3.1 基线资料
        收集患者年龄、性别、斜视类型、视力、屈光度、斜视度、眼位、眼球运动、视功能、手术肌肉条数等检查资料。其中视力和屈光状态检查采用E字 国际标准视力表(Early Treatment Diabetic Retinopathy Study Chart,简称ETDRS表),所有受试者术前均进行屈光检查,年龄<16岁受试者首次就诊需行睫状肌麻痹验光,参考中国儿童睫状肌麻痹验光及安全用药专家共识[12]和我国儿童屈光矫正专家共识[13];斜视度检查使用三棱镜交替遮盖法分别测量第一眼位6 m视标的斜视度和原在位注视33 cm调节性视标的斜视度;眼球运动检查分别在9个诊断眼位进行检查;双眼视功能采用同视机,立体视本和Worth四孔灯评估。通过Titmus立体图(Stereo Optical Inc., 美国)在33 cm检查立体视,结果以logMAR值记录;看远视功能通过Randot在6 m检查,分别记录联合眼罩组和眼垫组受试者的术前1 d和术后1 d的看近与看远视功能。
    1.3.2 术后指标
         主要结局指标:视功能相关生活质量采用美国国立眼科研究所视觉功能调查问卷-25(National Eye Institute 25-item visual function questionnaire,简称NEI-VFQ-25)问卷进行评价[14-15],该问卷包含3个部分:一般健康及视功能、活动时困难程度、视力问题的反映,共25个条目,每个条目分别为0~5分,总分0~125分,总分越高,代表视功能相关生活质量越好。
         次要结局指标:1)术后1 d的睡眠质量,睡眠质量采用Likert 5级评分[16],评分为0~4分,分数越高,代表睡眠质量越差。2)疼痛评分,本研究采用数字评价量表(numeric rating scale,简称NRS[17,18])对术后1 d眼部疼痛进行量化评估,评分为0~10分,分数越高,说明疼痛程度越严重。3)术眼评估,术后1 d对眼垫的渗血渗液情况及眼睑周围进行评估,并在裂隙灯下检查球结膜充血、水肿、手术切口外观和斜视改善程度。其中球结膜充血和水肿评分为0~4分,0分表示无充血或水肿,4分表示严重充血或水肿,分数越高,代表充血或水肿的程度越严重。手术切口外观和斜视改善程度评分为0~4分,分数越高,代表切口外观或斜视改善程度越好,其中0分表示切口外观极差或斜视无改善;4分表示切口外观非常好或斜视完全改善。4)术后1周的切口外观和患者满意度。满意度采用Likert 5级评分,评分为0~4分,评分越高,表明患者满意度越高。上述主要结局指标及术后1 d的次要结局指标评价均在患者术后第一天早晨拆除眼垫、眼罩和清洁后进行,术后1周情况在患者返院复查时进行,对于6~8岁儿童患者,其疼痛、入睡情况、视功能情况和满意度指标的评价由本研究经过培训的调查员,根据患儿的核心表述,在其家长的协助下共同完成,确保评价结果的准确性与可靠性。

1.4 统计学方法  

        基于主要结局指标计算样本量。假设联合眼罩组平均提升视功能相关生活质量评分10.0分、两个组的标准差均为15.0、显著性水平(α)设定为双侧0.05的条件下,采用两独立样本t检验,每组样本量为37例的两组样本能够达到80%的检验效能拒绝均值相等的零假设。预期5%的患者失访,最终需要样本量78例,每组39例。样本量使用PASS 16软件进行估算。
        采用SPSS 24.0统计软件分析数据。分组描述基线特征,并实施统计检验比较组间差异。计量资料以均值±标准差表示,组间比较采用独立样本t检验;同一指标的自身干预前后对比采用配对t检验;计数资料以频数和百分比表示,组间比较采用χ2检验。主要和次要结局指标的组间比较采用独立样本t检验,并计算组间差异均值及其95%置信区间(confidence interval,简称CI)。计量资料分布的正态性通过Shapiro-Wilk检验和直方图联合判断。统计均采用双侧检验,以P<0.05为差异具有统计学意义。

2 结果

2.1 一般资料情况

        联合眼罩组(一眼眼垫+一眼眼罩遮盖)纳入女性22例, 男性18例,年龄范围在6~52岁,平均年龄为(23.85±12.38)岁,共同性斜视15例,非共同性斜视25例,患者斜视手术肌肉线条为2~6条,平均为(3.33±0.76)条;眼垫组(双眼眼垫遮盖)纳入女性24例,男性16例,年龄范围在6〜56岁,平均年龄为(27.75±12.83)岁,共同性斜视17例,非共同性斜视23例,患者斜视手术肌肉线条为2~6条,平均为(3.20±0.79)条。两组患者的年龄、性别、斜视类型和斜视手术肌肉线条的分布差异均无统计学意义(P > 0.05)。见表1。

2.2 两组患者术后各项指标得分比较

        联合眼罩组患者在术后1 d的视功能质量总体得分高于眼垫组(均值差:7.28, 95% CI:0.25~1.31,P=0.043),其中视力问题的反映维度得分高于眼垫组(均值差:3.43, 95% CI:0.42~6.44,P=0.027);睡眠质量得分低于眼垫组(均值差:-0.33, 95% CI:-0.63~ -0.03,P=0.034)。联合眼罩组与眼垫组在术后1 d的NRS评分、球结膜充血、手术切口外观及斜视改善程度的比较,差异均无统计学意义(P>0.05)。术后1周,两组患者的切口外观得分均较术后1 d提升(P<0.001),联合眼罩组患者的满意度得分高于眼垫组(均值差:0.21, 95% CI:0.10~0.32,P<0.001)见表2。

3 讨论

        在双眼斜视矫正手术后,临床常规采用非透明医用眼垫完全覆盖患者双眼,以预防术眼细菌感染、限制眼球运动并避免患者揉眼。然而,此种遮盖方法因完全阻隔光线而易引发“黑暗恐惧效应”,尤其可能加重儿童患者的术后焦虑情绪与心理创伤[6]。既往研究报告,术后处于黑暗环境是引起患儿恐惧和躁动的重要因素之一,可导致其配合度下降,影响治疗,甚至引发并发症[19]。有研究结果表明,应用透光眼罩有助于预防小儿双眼斜视术后躁动的发生[20]。此外,双眼同时被遮蔽会影响患者术后必要的日常生活能力,对日间手术患者的恢复尤为不利。本研究结果发现,对双眼手术患者,采用“一眼眼罩联合一眼眼垫”的改良包眼方式能够在保障手术安全性的同时,减轻对患者术后生活的影响,为优化双眼斜视矫正术后护理提供可靠依据。
        随着斜视诊疗的发展,临床指南日益强调应在改善患者眼部功能的基础上,全面关注患者的生活质量,包括心理与社会适应能力[21,22,23,24,25]。本研究通过视功能问卷调查结果显示,联合眼罩组在视力问题的反映和总体视功能质量的得分高于眼垫组,表明改良遮眼方式在提升患者术后生活便利性(如自主行动、日常感知),并在维护精神心理健康方面表现出明显优势。尽管多孔眼罩提供的视力有限,但足以让患者感知到一定程度的环境、障碍物和光源。这种最低限度的环境感知能力增强了患者的自主性和控制感,减少了患者活动的困难程度,因此在视功能质量的“活动时困难程度”维度上,联合眼罩组得分较高。与双眼眼垫遮盖常导致行动受限相比,部分视觉保留减少了患者对他人照护的依赖,更符合日间手术患者对快速康复的需求,也有助于缓解因视力障碍引起的负面情绪。该结果与朱文卿等[6]的研究发现一致,其报道显示斜视矫正术后双眼使用透明眼罩遮眼比使用医用纱布能更  好地减少患者的焦虑 ,进一步证实不同遮盖方式可能对患者视觉功能恢复和心理状态产生不同影响。因此,临床上有必要更进一步探索并推广能够提升术后身心体验的遮盖方法。
        本研究比较了两种遮盖方式对双眼斜视矫正术后恢复的影响,发现术后1 d联合眼罩组在睡眠质量上优于眼垫组(P<0.05),提示该改良方式可能有效缓解患者术后不适感,特别是在情绪状态方面具有积极意义。从操作特点来看,眼垫组采用的双眼眼垫遮盖虽能为手术眼提供全面保护,但因完全阻断了视觉输入,可能导致患者出现空间定向障碍与安全感缺失,尤其易引起儿童或首次手术患者的焦虑情绪[8],从而影响睡眠质量。相比之下,联合眼罩组采用多小孔塑料眼罩具有“软硬适中”“多孔透气”的特点,将其用于遮盖手术肌肉少的眼睛或主导眼,允许光感和形觉穿透,使大脑皮质仍能接收信号,避免急性的、完全的视觉剥夺;另一眼虽用眼包遮盖,可在保护术眼的同时部分保留视功能,减轻因双眼同时遮蔽带来的心理冲击与视觉剥夺感,有助于维持对周围环境的判断能力,从而使患者放松与入睡。
        在术后满意度方面,术后1周两组患者均表现出较高满意度,联合眼罩组的满意度评分高于眼垫组(P<0.001),随着斜视日间手术的普及,最大限度维持视功能质量、减少术后早期不适、对患者安全返家及按期复查具有重要意义。本研究显示,改良遮盖方式更有助于提升日间手术患者的术后体验和整体舒适度,具备推广价值。此外,在眼部术后恢复指标方面,两组患者的疼痛、球结膜充血、切口外观(术后1 d和1周)均无差异,且两组患者术后1周的切口外观均优于术后1 d的切口外观,说明单眼眼罩遮盖并未额外增加眼部刺激或影响手术切口愈合,该结果可能与眼罩经过消毒灭菌和 “多孔设计” 的透气性密切相关,平衡了保护与舒适双重功能,既提供了物理屏障、避免外界污染,又能减少传统眼垫遮盖下眼周因闷热导致的不适。因此,改良的遮眼方案在提供心理和功能益处的同时,也保障了患者的安全性,故患者整体满意度提升。
        综上所述,对于双眼斜视矫正术后的患者,采用一眼眼罩联合一眼眼垫遮盖的改良遮盖方式能够通过减轻视觉剥夺,缓解恐惧和心理不适,进而改善患者视功能质量、睡眠质量和提升患者满意度。在保证术后恢复效果与安全性的前提下,该护理策略值得在临床上推广应用。本研究不足之处在于,这是一项非随机对照设计研究,由于无法完全控制混杂因素,可能引入选择偏倚或混淆偏倚,从而削弱研究结果的可信度和因果推断的强度。未来研究建议采用多中心随机对照试验设计,并通过分配隐藏和分层随机等方法控制潜在混杂变量,以进一步验证本研究的结论。
