随着人工智能(artificial intelligence,AI)技术的快速发展,基于深度学习(deep learning,DL)和机器学习的AI技术在医学领域上的应用受到了广泛的关注。AI在眼科的应用也逐渐向更全面更深入的层次发展,通过角膜断层扫描、光学相干断层扫描、裂隙灯图像等技术,AI在对角膜病变、结膜病变、白内障、青光眼等眼部疾病的诊断和治疗方面都表现出了良好的性能。然而AI在眼科的应用方面也存在一些诸如结果可解释性的欠缺、数据集标准化的缺乏、数据集质量的不齐、模型适用性的不足和伦理问题等挑战。在5G和远程医疗飞速发展的时代,眼科AI同时也有许多新的机遇。本文综述了AI在前段眼科疾病中的应用、临床实施的潜在挑战和前景,为AI在眼科领域的进一步发展提供参考信息。
With the rapid development of artificial intelligence (AI) technology, the application of AI technology based on deep learning (DL) and machine learning (ML) in the medical field has received widespread attention. The application of AI in ophthalmology is gradually being shifted to a more comprehensive and in-depth level. Trained on corneal tomography, optical coherence tomography (OCT), slit-lamp images, and other techniques. AI can achieve robust performance in the diagnosis and treatment of corneal lesions, conjunctival lesions, cataract, glaucoma and other ophthalmic diseases. However, there are also some challenges in the application of AI in ophthalmology, including the lack of interpretability of results, lack of standardization of data sets, uneven quality of data sets, insufficient applicability of models and ethical issues. In the era of 5G and telemedicine, there are also many new opportunities for ophthalmic AI. In this review, we provided a summary of the state-of-the-art AI application in anterior segment ophthalmic diseases, potential challenges in clinical implementation and its development prospects, and provides reference information for the further development of artificial intelligence in the field of ophthalmology.

迟发性眼前节毒性综合征(toxic anterior segment syndrome,TASS)是TASS的一种特殊类型,两者既有共同点又有区别,随着人们认识的深入,越来越多的迟发性TASS被报道。迟发性TASS较典型性TASS少见,流行病学特征因人工晶体不同而有差异。几乎所有可引起典型性TASS的因素都可以引起迟发性TA SS,此外铝元素在迟发性TA SS的发生中可能有关键作用。迟发性TA SS临床表现与典型性TASS类似,累及后段更为常见,激素治疗有效。
Late-onset toxic anterior segment syndrome (TASS) is a special type of TASS, which has both similarities and differences. With the deepening understanding, more and more late-onset TASS is reported. Late-onset TASS is less common than typical TASS, and the epidemiological characteristics are vary with intraocular lenses. Almost all the factors that can cause typical TASS can cause late-onset TASS, in addition, aluminum element may play a key role in the occurrence of delayed TASS. The clinical manifestations of late-onset TASS are similar to those of typical TASS, with the involvement of the posterior segment more common and hormone therapy effective.

光学相干断层扫描血管成像(optical coherence tomography angiography,OCTA)是一种非接触、无创的、快速的血管造影技术,它通过检测红细胞在血管内的光学相干断层扫描(optical coherence tomography,OCT)信号的变化来提供血管影像。由于这项技术不需要注射血管造影剂,使得它比传统的眼科血管造影技术更安全。在前节疾病的检查中,OCTA不仅能够量化前节血管的范围和密度,还显示出良好的成像质量,为临床监测疾病的病程和对治疗的反应提供客观的评价。
Optical coherence tomography angiography (OCTA) is a noncontact, noninvasive, and rapid angiography technique. It provides vascular images by detecting changes in the optical coherence tomography (OCT) signal of red blood cells in the blood vessel. Since this technique does not require injection of angiography, it is safer than traditional ophthalmic angiography. In the examination of anterior segment diseases, OCTA can not only quantify the range and density of anterior segment blood vessels, but also shows good imaging quality, providing an objective evaluation for clinical monitoring of the disease course and response to treatment.

光学相干断层扫描血管成像术(optical coherence tomography angiography,OCTA)作为一项新兴的检查手段,现已被广泛应用于眼科临床及科研工作中。鉴于OCTA在血管成像方面的独特优势,近年来关于其在眼前段的临床应用和研究也愈发得到关注。关于OCTA对结膜、巩膜、角膜以及虹膜的血管和相关眼表疾病的研究日益增多,其在眼前段应用潜力巨大。
As a new examination method, optical coherence tomography angiography (OCTA) has been increasingly used in ophthalmology clinical work and scientific research. In view of the unique advantages of OCTA in angiography,the clinical application and research of OCTA in the anterior segment have attracted more and more attention in recent years. Studies of OCTA in vessels of conjunctiva, sclera, cornea, iris, and related ocular surface diseases have shown great potential for its application in the anterior segment.

慢性闭角型青光眼小梁切除术后早期眼前节及屈光变化的临床研究

目的:观察并分析慢性闭角型青光眼患者小梁切除术后早期眼前节及屈光状态变化。方法:采用前瞻性研究方法,收集青光眼小梁切除手术病例20例20只眼,分别测定术前、术后1周、1个月及3个月的球面镜度数、角膜曲率、眼轴长度、前房深度,并进行统计学分析。结果:小梁切除术后1周球面镜度数较手术前差异均具有统计学意义(P<0.05),差值为(?1.20±0.55) D;术后1个月、3个月球面镜度数较手术前差异无统计学意义(P>0.05)。术前、术后1周、1个月垂直方向角膜曲率分别为(44.89±1.20) D、(45.72±1.54) D、(45.65±1.35) D,术后1周、1个月垂直方向角膜曲率较手术前显著性增加,差异具有统计学意义(P<0.05);术后3个月垂直方向角膜曲率较术后1个月差异无统计学意义(P>0.05)。术前眼轴长度(22.49±0.43) mm和前房深度(2.75±0.45) mm与术后1周眼轴长度(21.60±0.59) mm和前房深度(2.25±0.34) mm比较差异具有统计学意义 (P<0.01);术后1个月和3个月前房深度、眼轴长度较手术前差异无统计学意义(P>0.05)。结论:小梁切除术后早期(术后1个月)可发生球面镜度数增加,前房变浅、眼轴变短、垂直方向曲率增加的眼前节和屈光改变。从而发生近视飘移、而影响术后视力。随时间推移(术后3个月),前房加深、眼轴长度逐渐增加、垂直散光和近视飘移度数逐渐减少,眼前节和屈光变化逐渐趋于稳定,术后视力逐渐恢复。
Objective: To observe and analyze the changes of anterior segment and refractive state in patients with chronic angle closure glaucoma after trabeculectomy. Methods: It was a prospective study. A total of 20 cases with 20 eyes that have met the inclusion criteria undergoing trabeculectomy were collected. The diopter of spherical power,corneal curvature, axial length (AL) and anterior chamber depth (ACD) were measured before the surgery and 1 week, 1 month, and 3 months after the surgery respectively. Results: There was significant difference (P<0.05)for the diopter of spherical power before and 1 week after the trabeculectomy, the difference was (?1.20±0.55) D.After 1 month and 3 months, the diopter of spherical power was not statistically significant (P>0.05). The corneal curvature in the vertical direction was (44.89±1.20) D, (45.72±1.54) D, (45.65±1.35) before, 1 week and 1 month after operation. The corneal curvature in the vertical direction increased significantly at 1 week and 1 month after operation (P<0.05), and there was no significant difference (P>0.05) after 3 months. The preoperative mean ocular axis length (22.49±0.43 mm) and mean anterior chamber depth (2.75±0.45 mm) compared with postoperative mean ocular axis length (21.60±0.59 mm) and mean anterior chamber depth (2.25±0.34 mm) were statistically significant (P<0.01). Comparing 1 month and 3 months after operation, there was no significant difference in the depth of the anterior chamber and the length of the eye axis (P>0.05).Conclusion: In the early stage after trabeculectomy (1 month after operation), the diopter of spherical power can be increased. The ACD became shallower, the eye axis became shorter, and the curvature of vertical direction increased resulting in myopic drift,which affected postoperative visual acuity. Over time (3 months after operation), the anterior chamber deepened,the axial length gradually increased, the vertical astigmatism and myopia drift degree gradually decreased, the anterior segment and refractive changes gradually stabilized, and the postoperative visual acuity gradually recovered.
