Using a rigid lens as endocapsular supporting device in cataract surgery for moderate subluxated cataracts

Background: To present a surgical technique using a rigid intraocular lens as endocapsular supporting device in manual small incision cataract surgery (MSICS) for treating mild-moderate subluxated cataracts.

Methods: In our technique, a single-piece rigid polymethyl methacrylate (PMMA) lens was implanted in the bag following the nucleus removal, with its axis vertical to the zonular dialysis. This endocapsular-implanted IOL stretched the bag and provided sufficient stability and lens centration. This technique was performed in 19 eyes with subluxated cataracts, with zonulysis of ≤120 degree and nuclear sclerosis of grade ≤3. Mean follow-up time was 9.8 months.

Results: All eyes had endocapsular IOL implantation during surgery. Intraoperative extension of the dialysis did not occur in any eye. The IOL was placed in the bag in all but 1 case, in which dislocation of the IOL haptic into the vitreous occurred. Though the IOL was slightly decentered in 3 cases, it kept stable. All patients were asymptomatic.

Conclusions: This approach provides a simplified and practical strategy for surgically managing subluxation with mild-moderate zonular loss.

Visual Impairment and Rehabilitation

AB105. Database of retinal images in visually impaired individuals: drusen and age-related macular degeneration (AMD)

Background: With a large portion of older adults living longer, the number of individuals diagnosed with low vision is increasing. The use of optical coherence tomography/scanning laser ophthalmoscope (OCT/SLO) to diagnose retinal disease has become common place in the last 10 years, yet currently there are no OCT/SLO databases for pathological vision. Our aim is to develop a clinical database of individuals who have drusen (i.e., lipid deposits found under the retina), or have been diagnosed with age-related macular degeneration (AMD), with information as to how the structure of the diseased retina changes over time, as well as measures of visual and cognitive functional performance.

Methods: Fundus photographs and retinal scans will be taken using the same model of optos OCT/SLO located in three test sites (MAB-Mackay Rehabilitation Centre, School of Optometry Clinic at the University of Montreal, and the Lighthouse Institute, New York, USA). For each individual entry in the database, demographic and diagnosis information will be available. All OCT/SLO images will be graded according to the Age-related Eye Disease Study standard, in addition to number and size of drusen, severity of geographic atrophy, severity of pigment mottling and presence of choroidal neovascularization. Retinal topography and Raster scans from the OCT/SLO will provide a cross-sectional look at affected retinas. Fixation stability will be recorded using the SLO function, and present four different tasks that are designed to reproduce typical tasks of daily vision, with each task lasting for 10 seconds. The tasks are cross fixation, face recognition, visual search, and reading. These tasks in addition to the retinal scans will be used to determine the eccentricity of a preferred retinal locus from the anatomical fovea, and can be used as an outcome measure for clinical interventions in visually impaired patients.

Results: The database will be available to professors training eye-care practitioners and rehabilitation specialists as a teaching tool. Students will be able to familiarize themselves with the retina and a variety of AMD-related pathologies before they start working with patients. The database will also be accessible by researchers interested in studying AMD from basic science to epidemiology, to investigate how drusen and AMD impact visual and cognitive functional performance.

Conclusions: The common infrastructure is easily accessible to all VHRN members on request. The database will also be accessible online in 2018 (see http://cvl.concordia.ca for more information).

AB096. Neurophysiological measures of stigma stereotypes

Background: The perceptions surrounding assistive technology have been shown to be increasingly stigmatizing in older adult populations. This stigmatization can lead individuals to the abandonment of the assistive device. Until now, the methods of identifying or predicting the stigma surrounding assistive technology has mostly been qualitative in nature. Here we present a novel quantitate and qualitative research study that uses neuro-cognitive (psychophysics and EEG) and eye tracking technology, in addition to a new questionnaire to investigate the stigma associated with assistive devices. Therefore, this approach plays a major role in understanding and predicting the neural and physiological correlates associated to stigma.

Methods: Thirty-four older adults (>50 years) took part in the study. To determine the psychophysiological predictors of stigma surrounding assistive technologies, we monitored brain activity using EEG, heart rate and eye movements using an eye-tracker while participants viewed a series of images containing either an older or younger individual in different social scenarios (e.g., talking to doctor, at coffee shop). In each scenario, the individual uses either no assistive device, a low stigmatizing device (e.g., iPad), or a high stigmatizing device (e.g., electronic magnifier).

Results: Here we present preliminary analysis of the eye movement data. Analysis shows that in comparison to images that contained a low stigmatizing device, in images that contain high stigmatizing devices, the latency to fixate the device is shorter, first fixation duration is longer, and the total number of fixations on the device are higher. The environment that the devices is used in has no effect on eye movement metrics.

Conclusions: Although the sample size is small, and based on a healthy older-adult population, these initial observations would indicate that latency to fixate and first fixation duration are predictors of stigma associated with assistive devices. Future research should expand this prediction to those actively using assistive devices, and how the measures predict abandonment over time.

AB068. Texture density aftereffect can be bidirectional

Background: It has been suggested that adaptation to texture density only ever reduces, i.e., never increases, perceived density, implying that density adaptation is ‘uni-directional’ and that texture density is coded as a scalar attribute (Durgin & Huk, 1997). However, we have recently shown that simultaneous density contrast, which describes the effect of a surround texture on the perceived density of a centre region, is ‘bi-directional’—that is, not only do denser surrounds reduce perceived density of the center but sparser surrounds enhance it (Sun, Baker, & Kingdom, 2016). Therefore, we decided to re-examine the directionality of density adaptation.

Methods: We measured the density aftereffect in random dot patterns using a 2AFC matching procedure that established a point-of-subjective-equality (PSE) between an adapted test patch and an unadapted match patch. The adaptors and test were presented at the same position, either at top left or bottom right of the fixation. The match was presented at bottom left or top right correspondingly. These positions were fixed within a block and switched between blocks. Then, using sequential presentation, we measured the density aftereffect for a wide range of adaptor and test densities.

Results: In the first experiment, we observed a unidirectional density aftereffect when test and match were presented simultaneously as in previous studies. However, when they were presented sequentially, bidirectionality was obtained. This bidirectional aftereffect remained when the presentation order of test and match was reversed (second experiment). In the third experiment, we used sequential presentation to measure the density aftereffect for a wide range of adaptor densities (0–73 dots/deg2) and test densities (1.6, 6.4, and 25.6 dots/deg2). We found bidirectionality for all combinations of adaptor and test densities, consistent with our previous SDC results.

Conclusions: In three experiments, we found that density adaptation is bidirectional when the test and match stimuli are presented sequentially. The unidirectional density adaptation reported in previous studies might have been due to effects arising from simultaneous presentation of test and match stimuli. Our evidence again supports the idea that there are density-selective channels in the visual system in line with our previous finding in SDC.

AB064. Product knowledge predicts greater willingness to buy and gaze-related attention, salience does not

Background: Visual salience computed using algorithmic procedures have been shown to predict eye-movements in a number of contexts. However, despite calls to incorporate computationally-defined visual salience metrics as a means of assessing the effectiveness of advertisements, few studies have incorporated these techniques in a marketing context. The present study sought to determine the impact of visual salience and knowledge of a brand on eye-movement patterns and buying preferences.

Methods: Participants (N=38) were presented with 54 pairs of products presented on the left and right sides of a blank white screen. For each pair, one product was a known North American product, such as Fresca?, and one was an unknown British product of the same category, such as Irn Bru?. Participants were asked to select which product they would prefer to buy while their eye movements were recorded. Salience was computed using Itti & Koch’s [2001] computational model of bottom-up salience. Products were defined as highly salient if the majority of the first five predicted fixations were in the region of the product.

Results: Results showed that participants were much more likely to prefer to buy known products, and tentative evidence suggests that participants had longer total dwell times when looking at unknown products. Salience appears to have had little or no effect on preference for a product, nor did it predict total dwell time or time to first fixation. There also appears to be no interaction between knowledge of a product and visual salience on any of the measures analyzed.

Conclusions: The results indicate that product salience may not be a useful predictor of attention under the constraints of the present experiment. Future studies could use a different operational definition of visual salience which might be more predictive of visual attention. Furthermore, a more fine-grained analysis of product familiarity based on survey data may reveal patterns obscured by the definitional constraints of the present study.

AB063. Contrasting effects of exogenous attention on saccades and reaches

Background: The goal of the present study was to determine whether exogenous attentional mechanisms involved in motor planning for saccades and reaches are the same for both effectors or are independent for each effector. We compared how eye and arm movement parameters, notably reaction time and amplitude, are affected by modulating exogenous attentional visual cues at different locations relative to a target.

Methods: Thirteen participants (M =22.8, SD =1.5) were asked to perform a task involving exogenous attentional allocation and movement planning. The participants were asked to fixate and maintain their hand at an initial position on a screen in front of them (left or right of screen centre) and then, at the disappearance of the fixation cross, perform an eye or arm movement, or both, to a target square (mirror location of fixation cross). A distractor appeared momentarily just before the appearance of the target at one of seven equidistant locations on the horizontal meridian. Saccade reaction times (SRTs), reach reaction times (RRTs) and amplitudes were calculated.

Results: Compared to the neutral condition (where no distractor was presented), distractors overall did not result in a facilitation of SRTs at any location (shorter SRTs), rather only a strong inhibition (longer SRTs) as a function of distractor target distance. In contrast, RRTs showed strong facilitation at the target location and less inhibition at further distances. However, both SRTs and RRTs followed a similar pattern in that RTs were shortest closer to the target position and were increasingly longer as a function of distractor target distance. In terms of amplitude, there was no effect of the distractor on reach endpoints, whereas, for saccades, there was an averaging effect of distractor position on saccade endpoints, but only for saccades with short SRTs. These effects were similar when either effector movement was performed alone or together.

Conclusions: These findings suggest that attentional selection mechanisms have both similar and differential effects on motor planning depending on the effectors used, providing evidence for both effector independent and effector dependent attentional selection mechanisms. This study furthers understanding of the operating mechanisms of exogenous attention on eye and arm movements and the interaction between sensory and motor systems.

AB055. Eye movements in the dark: saccades to non-visual targets

Background: Saccades are rapid and abrupt eye movements that allow us to change the point of fixation very quickly. Saccades are generally made to visual points of interest, but we can also saccade to non-visual objects that attract our attention. While there is a plethora of studies investigating saccadic eye movements to visual targets, there is very little evidence of how eye movement planning occurs when individuals are performing eye movements to non-visual targets across different sensory modalities.

Methods: Fifteen adults with normal, or corrected to normal, vision made saccades to either visual, auditory, tactile or proprioceptive targets. In the auditory condition a speaker was positioned at one of eight locations along a circle surrounding a central fixation point. In the proprioceptive condition the participant’s finger was placed at one of the eight locations. In the tactile condition participants were touched on their right forearm in one of four eccentric location, left and right of a central point. Eye movements were made in complete darkness.

Results: We compared the precision and accuracy of the eye movements to tactile, proprioceptive, and auditory targets in the dark. Overall, both precision and accuracy of movements to non-visual targets were significantly lower compared to visual targets.

Conclusions: These differences emphasize the central role of the visual system in saccade planning.

Retina and Posterior Segment

AB016. A standardized approach to correlating OCT images to histopathology using paraffin embedded specimens: clarification of the ellipsoid zone and new opportunities

Background: The aim of this project is to develop a new standardized and cost-efficient method to compare optical coherence tomography (OCT) scans to their corresponding paraffin embedded histopathology sections in post-mortem eyes. This correlation will clarify the interpretation of OCT images, and it will also enable direct immunohistochemical characterization of features observed on OCT.

Methods: Study design: donor eyes were obtained from two separate eye banks. In order to minimize post-mortem change like retinal detachment and vitreous opacification, the eyes were fixed in a previously tested fixative solution. Time between death and fixation has been kept under 6 hours. Methods: Using a customized imaging device, nine post-mortem eyes were imaged with a SD-OCT machine. Subsequently, an 8mm trephine was used to isolate a portion of the posterior pole including the macular area and the optic nerve head for histopathological analysis. Paraffin embedded cross sections of the retina were obtained and visually compared to each OCT image (b-scans).

Results: To facilitate the correlation of OCT images to their histopathological sections, three principle aspects were controlled during tissue processing: rotation, tilt and location. Using markings as well as anatomical landmarks, serial histopathological sections in an orientation comparable to OCT b-scans were obtained, thereby facilitating image pairing.

Conclusions: Compared to other well-established methods using resin and electron microscopy, our standardized Methods allowed us to successfully compare OCT b-scans to serial retinal cross sections of a wider macular area at a lower cost. Our novel approach allows us to translate features observed on OCT images into well-established histopathological images, providing the clinician with additional tools to obtain difficult diagnoses with more confidence.

Socket discomfort in anophthalmic patients—reasons and therapy options

Abstract: A smooth and timely fitting of a visually appealing, custom-made eye prosthesis after the loss of an eye is not only essential from a cosmetic point of view but above all facilitates good social and psychological rehabilitation. Cryolite glass prostheses must be replaced at least once a year, PMMA prostheses polished once a year and renewed every five years. In children, especially in growth phases, the fit of the prosthesis should be checked at least every six months and adjusted, if necessary. Ocularists and ophthalmologists should determine an individual cleaning procedure together with the patient, which depends on both the prosthesis material and external factors. Complications such as allergic, giant papillary, viral, and bacterial conjunctivitis or even blepharoconjunctivitis sicca must be detected and treated at an early stage to avoid discomfort and to maintain the ability of prosthesis wear. In the case of inflammation-induced shrinkage of the conjunctival fornices or post-enucleation socket syndrome, surgical interventions are necessary. In summary, an early supply with an eye prosthesis, adequate treatment of complications, and attention to psychological aspects, form the basis for a successful long-term rehabilitation of anophthalmic patients.
