Background: This study aimed to explore the best screening measure of myopia and its associated optimal referral criteria for children aged between 6 and 16.
Methods: After informed consent of cycloplegia was obtained, a total of 6,321 children aged between 6 and 16 from 53 primary or junior middle schools in Jiading District of Shanghai were included in our populationbased study. Receiver operating characteristic (ROC) curve was performed to determine the best cutoff points, sensitivities and specifi cities of diff erent screening tests, including uncorrected visual acuity (UCVA), noncycloplegic autorefraction (NCAR) and the combination of these two tests.
Results: There was significant difference in prevalence of myopia between boys and girls (χ2 =6.358, P=0.012). Compared with children of low age, the prevalence of older children was significantly higher (χ2 =1,386.404, P<0.001). For the combination of UCVA and NCAR, the best cutoff point was UCVA less than or equal to 0.2 logarithm of minimum angle of resolution (MAR) (20/30) and NCAR spherical equivalent refraction (SER) less than or equal to ?0.75 diopters (D), with associated sensitivity and specifi city of 75.0% and 85.0%, respectively, which were higher than those of UCVA and NCAR alone. After stratified by age, the best cutoff points were varied among children. The accuracy for children aged between 9 and 12 was higher than that for other ages.
Conclusions: The best screening measure of myopia was the combination of UCVA and NCAR. The optimal referral criteria of myopia for children should be age-determined.

形觉剥夺性和光学离焦性近视豚鼠视紫红质表达变化研究

目的:研究形觉剥夺性和光学离焦性近视豚鼠视紫红质的表达变化,探讨视紫红质表达与实验性近视眼之间的关系。
方法:40只出生后1周的豚鼠随机分为形觉剥夺组和光学离焦组(n=20),形觉剥夺组单眼戴半透明(半透明薄膜贴于平镜表面)平光硬性角膜接触镜片(Rigidgass-pemmeable contactlens, RCP),光学离焦组单眼藏-4.0DRGP镜片,另一只眼为对照组。戴镜干预后1、2周各组分别测量屈光度、眼轴长度、玻璃体腔深度,并于上午10~12 点钟取材,实时荧光定量PCR观察视紫红质 mRNA 的变化 Westem-blot 观察视紫红质的变化,进行对比比较,统计分析。
结果:实验干预后1周,形觉剥夺组和光学离焦组与对照组相比各项指标无显著性差(除形觉剥夺组屈光度以外)。实验干预后2周,与对照组相比较,形觉剥夺组和光学离焦组明显发生近视、眼轴延长、玻璃体腔加深(t=22.20、18.32、19.65、15.78、6.18、11.20.P<0.01):形觉剥夺组视紫红质及其 mRNA 表达均增加(t=17,489、14.31.P<0.05)光学离焦组视紫红质及其mRNA表达无明显变化。
结论:视紫红质的表达可能参与了形觉剥夺性近视眼的形成,而在光学离焦性近视眼中作用有限。

Purpose:To investigate the rhodopsin expression in form-deprived and defocus myopiain guinea pig and study the relationship between the rhodopsin expression andexperimental myopia.
Methods:Fourty guinea pigs were randomized into the form-deprived group and thedefocus group (n= 20 ). Guinea pigs in the form-deprived group wore a diffuser(rigidgass-permeable contact lens(RGP)on one eye since one week after birth. Those in defocus group wore a -4 D RGPon one eye. The contralateral eyes were left ascontrol. Refraction, axial length and depth of vitreous cavity were measured after 1and 2 weeks respectively. Retina were dissected at 10 ~ 12 o'clock in the moring.The level of rhodopsin and its mRNA were observed through Western-blot and real-time PCR respectively.
Result:There is no difference between form-deprived group, defocus group and controlgroups(except refraction in form-deprived group). One week later, there is nodifference between the form-deprived group, the defocus group and the control groups(except refraction in form-deprived group). Two weeks later, eyes in the form-deprivedgroup and the defocus group became myopic. Its axial length lengthened and depth ofvitreous cavity appeared deep. The form-deprived groups showed an increasedexpression of rhodopsin and its mRNA compared to the control groups. There is nodifference between the defocus group and the control groups.
Conclusion : Expression of rhodopsin might involve formation of form-deprived myopia,but has less influence on defocus myopia.

从基因层面揭示户外活动与近视的因果关系:基于孟德尔随机化原理

Revealing the causal relationship between outdoor activities and myopia from genetic level: based on Mendelian randomization

目的:运用孟德尔随机化(Mendelian randomization,MR)方法,探索户外活动与近视之间的双向因果关系。方法:来自英国生物银行(UK Biobank)的大型队列研究数据,选择与欧洲血统人群中户外活动与近视相关的相互独立的遗传位点作为IV。户外活动的全基因组关联研究(genome-wide association study, GWAS)数据包含419 314名欧洲人群,而近视的GWAS数据则包含460 536名欧洲人群,其中37 362名近视者和423 174名对照者。通过运用逆方差加权法(inverse variance weighted,IVW)、加权中位数法(weighted median,WM)以及MR Egger法进行MR分析,将比值比作为效应度量指标,深入探讨两者间的双向因果联系。同时,通过MR多态性残差和异常值检测(MR PRESSO)方法剔除SNP异常值,利用MR Egger法以及IVW法的Cochran Q检验对各个单核苷酸多态性(SNP)之间的异质性进行了评估;并且使用MR Egger截距检验SNP的潜在多效性,通过“留一法”敏感性分析检验MR研究是否受单个SNP的影响。结果:IVW分析显示户外活动能显著降低近视的风险(OR = 0.934, 95% CI: 0.922~0.948, P < 0.01)。反向孟德尔随机化分析发现近视者参与户外活动的意愿较低(OR = 0.925, 95%CI: 0.777~1.103)但P = 0.39,未达到统计学意义。双向孟德尔随机化分析的Cochran Q检验、MR PRESSO检测以及MR Egger截距测试结果均显示所选IV间不存在显著异质性和水平多效性问题,而且,“留一法”敏感性分析证实,单个SNP对整体结果未见影响。结论:户外活动可能明显降低近视的风险。
Objective: To employ Mendelian randomization (MR) methods to explore bidirectional causal relationships between outdoor activities and myopia. Methods: Large-scale cohort study data from the UK Biobank were utilized, selecting independent genetic loci associated with outdoor activities and myopia within the European ancestry population as instrumental variables. The outdoor activities GWAS data included 419,314 individuals of European descent, while the myopia GWAS data comprised 460,536 individuals, including 37,362 myopia cases and 423,174 controls. MR analyses were conducted using inverse variance-weighted (IVW), weighted median, and MR Egger methods, employing the odds ratio as the effect measure to thoroughly investigate bidirectional causal connections. Mendelian randomization pleiotropy residual sum and outlier (MR PRESSO) detection method were employed to eliminate SNP outliers. Cochran's Q test, within MR Egger and IVW methods, was utilized to assess heterogeneity among individual single nucleotide polymorphisms (SNPs). MR Egger intercept testing assessed potential pleiotropy, and sensitivity analysis using the "leave-one-out" method examined the influence of individual SNPs on overall results. Results: IVW analysis demonstrated that outdoor activities significantly reduce the risk of myopia (OR = 0.934, 95% CI: 0.922~0.948, P0.01). Reverse Mendelian randomization analysis revealed a non-significant lower propensity for myopic individuals to engage in outdoor activities (OR = 0.925, 95% CI: 0.777~1.103, P = 0.39). Cochran's Q test, MR PRESSO, and MR Egger intercept tests in bidirectional Mendelian randomization analysis all indicated no significant heterogeneity or horizontal pleiotropy issues among the selected instrumental variables. Furthermore, sensitivity analysis using the "leave-one-out" method confirmed that individual SNPs did not significantly impact the overall results. Conclusion: Outdoor activities significantly reduce the risk of myopia.

虚拟现实技术在儿童青少年近视视觉功能异常中的应用研究

目的:比较轻、中、重度近视患儿之间的视觉功能的差异,探索虚拟现实下的短期可塑训练对近视视觉功能的改善效果。方法:选择2022年6月—2022年9月就诊于苏州大学附属儿童医院的6~16岁儿童102例,按照屈光度分为正常对照组、轻度近视组和中重度近视组,进行眼科常规检查和视知觉功能检查,并进行视觉短期可塑训练。结果:近视患儿存在立体视功能缺损,近视程度与精细立体视功能损害呈正相关,各组之间比较差异具有统计学意义(P<0.05),部分患儿存在中心凹抑制。训练后,轻度近视组的中距离精细立体视功能得到改善,比较差异具有统计学意义(P<0.05)。结论:近视患儿存在立体视功能异常和中心凹抑制,且立体视功能的损害随着近视程度的增加而加重。轻度近视患儿部分精细立体视功能经过虚拟现实短期可塑训练可得到改善,而中重度近视患儿精细立体视功能改善不明显。
Objective: To compare the difference of visual function among children with myopia in different diopter, and explore the effect of short-term plastic training of virtual reality on visual function improvement. Methods: The 102 children aged 6-16 years who admitted to Children's Hospital of SoochowUniversity from June 2022 to September 2022 were recruited and divided into control group, mild myopia group and middle and high myopia group according to diopter. Routine ophthalmologic examination and visual perception function examination were carried out, and short-term plastic training was implemented. After the training, visual perception function examination was completed again. Results: There were significant differences in stereoscopic function defect at different distances among myopic children, a negative correlation between myopia and stereoscopic function was found. while there were several cases have foveal suppression. After training, the fine stereopsis at medium distance of the mild myopia group was significantly improved with significance statistical difference. Conclusions: Myopia can lead to the abnormality of stereopsis and foveal suppression in children. The defect of binocular visual function increases with the increase of myopia. Short term plastic training of virtual reality can partially improve the fine stereoscopic function of mild myopia children whileshowsnosigni ficanceimprovementof the fine stereoscopic function in middle and high myopia group.

近视对使用C字视力表与E字视力表检测视力的影响

The influence of myopia on visual acuity using C- and E-word visual acuity chart

目的:比较不同受检者在相同近视屈光欠矫下使用C字视力表和E字视力表所测得的视力差别及探讨导致这种差别的原因。方法:选取不同年龄段250名受检者,在完全矫正屈光度[最正之最佳视力(maximum plus to maximum visual acuity,MPMVA)]情况下附加相应的球镜造成相应的近视度数后比较使用2种不同的视力表测出的视力值变化情况。同时根据2种视力表的设计原理及视力表的不同记录法探讨造成数值不同的可能原因。结果:同一附加度情况下分别用C字视力表组和E字视力表组行方差分析,不同年龄组间差异无统计学意义(P>0.05)。相同附加度情况下同一受检者使用C字视力表组和E字视力表组行t检验,两者差异具有统计学意义(t=?15.798,P<0.05),C字视力表测得视力平均值小于E字视力表测得视力平均值。结论:受检者使用C字视力表比E字视力表检测视力结果更加优异;C字视力表和E字视力表的视标形状及开口方向不同是2种视力表检测结果差异的主要原因。
Objective: To compare the difference of visual acuity measured by C-chart and E-chart in the same myopic refractive undercorrection and to explore the causes of the difference. Methods: Two hundred and fifty subjects of different ages were selected to compare the changes of visual acuity measured by two different visual meters after adding corresponding spherical lenses to the maximum plus to maximum visual acuity (MPMVA) corrected diopter. At the same time, according to the design principle of the two kinds of visual acuity meters and the different recording methods of visual acuity tables, the possible reasons for the different values were discussed.Results: There was no significant difference between different age groups (P>0.05). Under the same degree of additionality, the visual acuity of the same subjects was tested by t-test with C visual acuity chart and E visual acuity chart. The difference was statistically significant (t=?15.798, P<0.05). The average visual acuity of C visual acuity table was smaller than that of E visual acuity table. Conclusion: The visual acuity of myopic patients tested by C visual acuity chart is better than by E visual acuity chart. The main reason for the difference between C visual chart and E visual chart was the different shape and opening direction of visual icons.

高度近视合并白内障患者术后有效晶状体位置的初步临床研究

目的:探讨高度近视合并白内障患者白内障术后有效晶状体位置的变化。方法:收集白内障超声乳化摘除联合人工晶状体植入术的高度近视合并白内障患者21例共27眼,记录术后1 d、1周、1个月视力、屈光度、有效晶状体位置。结果:高度近视合并白内障患者术后1 d,1周,1个月有效晶状体位置分别为(4.17±0.39),(4.09±0.38),(4.31±0.44) mm,且组间比较差异有统计学意义(P<0.05)。有效晶状体位置的实际轴向运动为(0.19±0.13) mm,与晶状体厚度呈正相关(r=0.648,P<0.001),与术前眼轴无相关关系(r=0.227,P=0.255)。结论:高度近视合并白内障患者术后1 d至1周人工晶状体轻度前移而造成近视偏移,而术后1个月时则呈远视偏移,且术后有效晶状体位置位移与术前晶状体厚度相关。
Objective: To investigate the change and influencing factors of the effective lens position after cataract surgery in high myopia eyes. Methods: We collected 27 eyes of 21 patients with high myopia who underwent phacoemulsification and intraocular lens implantation. The visual acuity, diopter and effective lens position were recorded 1 day, 1 week, and 1 month after operation. Results: The effective lens position of high myopia combined with cataract patients at 1 day, 1 week, and 1 month was (4.17±0.39), (4.09±0.38), and (4.31±0.44)mm, respectively. The effective lens positions significantly differed between the groups after surgery (P<0.05). The ELPRMS was (0.19±0.13) mm. The ELPRMS was positively correlated with the lens thickness (r=0.648,P<0.001), whereas was not correlated with the preoperative axial length (r=0.227, P=0.255). Conclusion: High myopic patients with cataract have a slight forward movement of the effective lens position from 1 day to 1 week after surgery, and a hyperopic shift from 1 week to 1 month. The displacement of effective lens position after surgery is correlated with the preoperative lens thickness.

病理性近视相对高度近视而言,更强调眼底并发症的存在。后巩膜葡萄肿(posterior staphyloma,PS)被认为是病理性近视的标志性特征,是眼球后巩膜壁的局部扩张,通常认为与巩膜变薄和脉络膜萎缩等因素相关。近年来研究认为P S的形成可能与局部炎症、Bruch膜缺失等因素密切相关。伴随OCT等检查技术的快速革新以及P S治疗手段的探索,诊断和治疗更加完善。
Pathological myopia emphasizes the existence of fundus complications. Posterior staphyloma, considered as a hallmark of pathological myopia, is a partial extension of the posterior scleral wall, which is believed to be related to the scleral thinning and choroidal atrophy. In recent years, studies have indicated that the formation of posterior staphyloma may closely connected with localized inflammation and defects of Bruch’s membrane.With the rapid innovation of examination technologies such as optical coherence tomography (OCT) and the exploration of treatment for posterior staphyloma. The diagnosis and therapy will be more comprehensive in the future.

近视是危害儿童青少年视力最常见的眼部疾病,高度近视对视功能造成极大的威胁。近年来,我国近视发病率逐年升高,对近视筛查与防控的需求也不断增加,随着人工智能理论与技术的不断发展与成熟,可以辅助眼科医生进行近视筛查、诊断与治疗。本文将简要介绍人工智能在近视的筛查、预测、检测、病理性近视以及角膜屈光手术中的应用,浅谈了目前人工智能在研究中存在的可比度较低、影像要求较高、可解释性较低及隐私保护等问题,并展望人工智能在近视相关领域的应用前景。
Myopia is the most common ocular disease that harms the vision of children and adolescents. High myopia poses a great threat to visual function. The incidence of myopia in China has been increasing in recent years, and the demand for myopia screening, prevention and control has also expanded. With the continuous development of artificial intelligence theory and technology, Artificial intelligence can assist ophthalmologists in myopia screening, diagnosis and treatment. This review will briefly introduce artificial intelligence in the screening, prediction, and detection of myopia; also, the application in pathological myopia and corneal refractive surgery. This review will discuss some problems of current artificial intelligence research, such as low comparability, high image requirements, low interpretability, privacy protection, and the application prospects of artificial intelligence in myopia.

佛山市禅城区小学五年级、六年级青少年近视情况及心理健康调查

目的:研究佛山市禅城区五年级、六年级近视儿童的心理健康及其影响因素,为解决其因近视产生的心理问题及近视防治工作提供依据。方法:选择佛山市禅城区3所小学,在其五年级、六年级选择2个班作为调查对象开展近视筛查、并使用儿童社交焦虑量表、孤独量表以及儿童自尊量表进行心理健康测量。结果:佛山市禅城区近视患病率52.0%,女性(57.5%)较男性(47.4%)有较高患病率,六年级患病率(57.6%)高于五年级(46.3%)。近视儿童均比正常视力儿童存在明显的社交焦虑、孤独感以及低自尊度(得分分别为5.53±4.04 vs 4.66±3.59,30.15±11.72 vs 27.29±10.40,97.97±12.73 vs 100.87±12.21),差异均具有统计学意义(均P<0.05)。进一步分析显示,初发近视社会焦虑、孤独以及自尊方面比中度近视明显。性别、年龄等社会因素对于近视儿童心理健康影响不明显。根据矫正的不同方式分类,近视儿童中,戴角膜塑形镜者社交焦虑感较低(F=4.21,P=0.01),戴框架眼镜者则较容易产生较低的自尊度(F=5.67,P=0.02)。结论:小学生近视问题是全社会普遍关心的问题,近视与其心理健康关系密切,这需要全社会密切配合,采取综合措施,且近视矫正方式的选择影响近视儿童的心理。
Objective: By studying the mental health and its influencing factors of myopia children in grade 5 and grade 6 in Chancheng District, Foshan City, this paper provided the basis for solving the psychological problems caused by myopia and the prevention and treatment of myopia. Methods: Three primary schools in Chancheng District,Foshan City were chosen. Two classes in grade 5 and grade 6 were selected to carry out myopia screening followed  by evaluation of their mental health with children’s social anxiety scale, loneliness scale and children’s self-esteem scale. Results: The prevalence of myopia was 52.0% in Chancheng District, Foshan City. Females (57.5%) had a higher prevalence than males (47.4%). The prevalence in grade 6 (57.6%) was higher than that in grade 5 (46.3%).Children with myopia had obvious social anxiety, loneliness, and low self-esteem (scores of 5.53±4.04 vs 4.66±3.59,30.15±11.72 vs 27.29±10.40, 97.97±12.73 vs 100.87±12.21, respectively) than those with normal vision, whose difference was statistically significant (all P<0.05). Further analysis showed that social anxiety, loneliness, and self-esteem in children with primary myopia were more obvious than those with moderate myopia. Social factors such as gender and age had little effect on the mental health of myopic children. According to different correction methods, among myopic children, those who wore orthokeratology lenses had lower social anxiety (F=4.21, P=0.01), and those who wore spectacles were more likely to have lower self-esteem (F=5.67, P=0.02).Conclusion: The problem of myopia in primary school students, which is closely related to mental health, is of
general concern to the whole society. This requires the whole society to cooperate closely and take comprehensive measures. The choice of myopia correction methods affects the psychology of myopic children.
