眼表菌群与眼表疾病关系的研究进展

眼表菌群是定植于眼表的各种微生物群落,以细菌为主。在正常情况下,眼表菌群与人体眼表组织的细胞和平共生,维持眼表的稳态,共同保证眼表的健康。但在环境改变或免疫力低下的情况下,眼表菌群会发生变化,与眼部疾病的产生与发展关系密切,对人类的健康造成巨大的危害。随着组学研究的不断发展,我们对眼表菌群有了新的认识,为眼表疾病的发病机制、治疗开辟了新的思路,同时也提出了新的挑战。本文对国内外眼表菌群与疾病关系进行综述,为眼表疾病的发生、发展以及治疗提供参考。
The microbiome of the ocular surface consists of various microbial communities that colonize on the eye surface, mainly bacteria. The stabilization of the microbiome and the other ocular surface components plays an important role in maintaining the homeostasis of the ocular surface. However, unpredictable changes of ocular surface microbiome are strongly associated with ocular surface diseases in the situation of environmental changes or destruction of immune system. With the innovation of inspection technology, the current gene sequencing technology is applied to detect the ocular surface microbiome and confirm that the eye microbiome is closely related to ocular surface diseases. This paper investigates the corelation of ocular surface microbiomes and diseases. Moreover, we provide areference for the occurrence and development of ocular surface diseases and their treatment.

城市颗粒物与眼表疾病及其潜在致病机制

越来越多的证据表明,空气污染可严重损害人体健康,成为全球疾病负担的主要原因。并且污染空气中的主要成分空气颗粒物可渗透到肺部和心血管系统,引起缺血性心脏病、肺炎、慢性阻塞性肺疾病甚至肺癌,导致空气污染相关的发病率和死亡率升高。城市颗粒物作为城市居住人群面临的主要空气污染问题,被证实与多种炎症性眼表疾病密切相关,如过敏性结膜炎、角膜炎、干眼等。高浓度城市颗粒物暴露还会引起睑板腺病理性结构改变和功能异常,诱发炎性睑板腺功能障碍。文章综述了城市颗粒物相关的眼表疾病类型及其致病机制的最新研究,旨在阐明空气污染对于眼表组织的损害和相应的潜在治疗靶点,指导临床上环境相关眼病的诊断与治疗。
More and more evidence indicate that air pollution can seriously damage human health and become a major cause of the global disease burden. The main component of air pollution, particulate matter, can penetrate the lungs and cardiovascular system, causing ischemic heart disease, pneumonia, chronic obstructive pulmonary disease, and even lung cancer, leading to an increase in the incidence and mortality rates related to air pollution. Urban particulate matter, as the main air pollution problem faced by urban residents, has been shown to be closely related to various inflammatory eye diseases, such as allergic conjunctivitis, keratitis, and dry eye. Our research further confirms that exposure to high concentration of urban particulate matter can also cause pathological structural changes and functional abnormalities in the meibomian gland, leading to inflammatory meibomian gland dysfunction. This review comprehensively summarizes the latest research on the eye surface diseases related to urban particulate matter and their pathogenic mechanisms, aims to elucidate the damage of air pollution to eye surface tissues, identify potential therapeutic targets, and guide the diagnosis and treatment of environmentally related eye diseases in clinical practice.

小牛血去蛋白提取物眼用制剂在眼表疾病应用中的回顾与展望

眼表是统一整体,依靠泪液、细胞、神经及免疫等综合因素维持平衡,某个或多个因素的失衡会造成眼表出现一系列连锁反应,导致眼表疾病,影响个体生活质量。近年来,越来越多研究围绕细胞能量代谢与多种疾病的发生、发展和治疗进行,纠正细胞能量代谢的异常是治疗此类疾病的重要思路。小牛血去蛋白提取物从发现并应用至今已有60多年历史,具有良好的组织修复作用和安全性。作为经典的能量补充修复剂,小牛血去蛋白提取物眼用制剂以特定工艺去蛋白处理的小牛血液提取物作为其主要有效成分,具有多重作用机制,在不同严重程度的角膜损伤、干眼症、眼烧伤等多种眼表疾病的治疗方面展现出良好的效果。文章就小牛血去蛋白提取物眼用制剂的制备工艺、作用机制、安全性以及临床应用等进行综述,旨在系统梳理小牛血去蛋白提取物眼用制剂的临床应用价值,并为其在未来的实验研究以及在眼表疾病的进一步实践应用提供科学依据和参考价值。
The ocular surface is an integrated entity maintained in equilibrium by a complex interplay of factors, including tears, cellular components, neural mechanisms, and immunological processes. Disruption of one or multiple factors can precipitate a cascade of reactions on the ocular surface, leading to ocular surface diseases and consequently affecting an individual's quality of life. In recent years, more researches have focused on the relationship between cellular energy metabolism and the occurrence, development, as well as treatment of a wide range of diseases. To treat such diseases, correcting the abnormality of cellular energy metabolism is an important idea. Deproteinized calf blood extract ophthalmic preparation has been discovered and applied over sixty years, demonstrating excellent tissue repairment and safety. As a classic energy supplement, deproteinized calf blood extract ophthalmic preparation process with specific technique involving deproteinized calf blood extract as its main active ingredient and showing significant effects in the treatment of various ocular surface diseases such as corneal injuries, dry eye syndrome, and ocular burns. This article summarized the preparation process, mechanism of action, safety, and clinical applications of deproteinized calf blood extract ophthalmic preparation. It aims to systematically organize the clinical utility of deproteinized calf blood extract ophthalmic preparation and provide scientific evidence and reference value for future research and practical applications in the treatment of ocular surface diseases.

甲状腺相关眼病眼表损伤的研究进展

甲状腺相关眼病是一种器官特异性自身免疫性疾病,其发病机制复杂,是成人最常见的眼眶疾病,且发病率逐年升高。该病不仅会导致眼球突出、眼睑退缩、睑裂扩大、眼球运动障碍等外观上的变化,且患者往往伴发眼红、眼痛、干涩、异物感、复视、视力下降等不适症状,其中眼表疾病的发病率较正常人明显增高,但其具体发病机制尚待进一步挖掘。目前研究表明甲状腺相关眼病患者的眼表损伤主要与眼表暴露增加、炎症侵犯、激素改变有关。角膜、结膜、泪膜、睑板腺等组织受累发生病理改变是患者表现出不同临床症状的直接原因。文章通过分析近年来国内外关于甲状腺相关眼病与眼表疾病等方面的相关研究,针对甲状腺相关眼病眼表损伤的病理改变及机制研究进展做一综述。
Tyroid-associated ophthalmopathy is an organ-specifc autoimmune disease with complex pathogenesis. It is the most common orbital disease in adults, and its incidence increases year by year. Tis disease can not only lead to appearance changes such as eyeball protrusion, eyelid retraction, eyelid cleat enlargement, eye movement disorders, and patients are ofen accompanied by red eyes, eye pain, dryness, foreign body sensation, diplopia, vision loss and other uncomfortable symptoms, among which the incidence of ocular surface diseases is signifcantly higher than that of normal people, but its specifc pathogenesis needs to be further explored. Current studies have shown that ocular surface injury in patients with thyroid-associated ophthalmopathy is mainly related to increased ocular surface exposure, infammatory invasion, and hormonal changes. Te pathological changes of cornea, conjunctiva, tear flm, meibomian gland and other tissues involved are the direct causes of different clinical symptoms of patients. In this paper, through the analysis of recent domestic and foreign studies on thyroid-associated ophthalmopathy and ocular surface diseases, the pathological changes and mechanism of ocular surface injury in thyroid-associated ophthalmopathy were reviewed.

儿童常见眼表疾病并发散光的相关研究进展

散光是儿童视力发育的重要威胁之一,很多情况下其发生与眼表疾病的进展密切相关。非先天性眼表疾病如睑缘炎相关性角结膜病变、春季角结膜炎、干眼、感染性角膜炎和眼外伤等可通过慢性炎症介导的眼表微环境失衡、角膜瘢痕产生、泪膜稳定性破坏及角膜生物力学结构异常重塑等途径,导致角膜表面不规则性的产生和角膜曲率的改变。先天性眼表疾病如圆锥角膜、睑内翻和眼表肿瘤等则可通过遗传或发育异常改变眼睑结构、改变角膜曲率的对称性和机械压迫角膜产生散光,例如遗传疾病圆锥角膜的进行性角膜变薄与发生春季角结膜炎时眼表微环境的炎症性改变可协同作用加剧散光进展。针对不同病因,局部糖皮质激素滴眼液、免疫抑制剂环孢素滴眼液等药物使用和手术治疗均可改善患儿本身的病情和散光情况,只是不同疾病的治疗和散光矫正方案不尽相同。现有研究虽指出了部分儿童眼表疾病与散光的关联,但对于儿童的特异性机制部分如发育过程中免疫系统的变化、角膜可塑性差异及各种因素交互作用效应尚未完全解析。
Astigmatism is a significant threat to children's visual development and is often closely linked to the progression of ocular surface diseases. Non-congenital conditions, such as blepharitis-associated keratoconjunctivitis, vernal keratoconjunctivitis, dry eye, infectious keratitis, and ocular trauma, can induce irregularities on the corneal surface and changes in corneal curvature. This occurs through mechanisms like the ocular surface microenvironment imbalance mediated by chronic inflammation, corneal scarring, tear film instability, and abnormal corneal biomechanical remodeling. Congenital disorders, including keratoconus, entropion, and ocular surface tumors, can also lead to astigmatism. These conditions do so by causing genetic or developmental abnormalities that alter the eyelid structure, disrupt corneal symmetry, or exert mechanical pressure. Notably, there can be synergistic effects between progressive corneal thinning seen in hereditary keratoconus and the inflammatory microenvironment changes associated with vernal keratoconjunctivitis. Therapeutic interventions, such as topical corticosteroids, cyclosporine eye drops, and surgical procedures, can improve both the primary conditions and the astigmatism. However, treatment strategies vary depending on the different etiologies. Current research has established associations between pediatric ocular surface diseases and astigmatism. Nevertheless, the specific developmental mechanisms, including the  maturation of the immune system, differences in corneal plasticity, and the interactive effects of multiple factors, remain incompletely understood.

蠕形螨感染相关眼表疾病的研究进展

蠕形螨是人类皮肤上最常见的寄生虫,在眼部主要寄居于毛囊、睑板腺及皮脂腺,可引起眼干、眼痒、异物感明显等眼部症状。目前已证实有2种寄生于人类的蠕形螨:毛囊蠕形螨和皮脂蠕形螨,二者均可诱发各类眼表疾病,如睑缘炎、睑板腺疾病、角膜病、翼状胬肉以及眼红斑痤疮等,但是由于疾病症状相似、检查遗漏以及认知不到位等主客观因素,该病易被误诊、漏诊。蠕形螨具有高度的年龄依赖性,并且可以在无症状的成年人中发现,因此蠕形螨的致病性一直存在争议,现有研究表明,蠕形螨可以通过直接损伤、诱发超敏反应和作为载体携带细菌等方式致病。蠕形螨的感染可以通过有效的手段进行检测,确诊后可通过热敷、眼睑清洁、局部或全身使用除螨药物进行治疗。了解蠕形螨在眼部疾病中的重要性对于准确诊断和适当的管理策略至关重要,近年来对于蠕形螨的研究越来越多,有必要对蠕形螨感染相关眼表疾病的诊断及治疗技术进行更新,因此本文综述了蠕形螨的病原学、流行病学、致病机制、检出方法,探讨了蠕形螨感染与各类眼表疾病之间的联系以及治疗方法,以期为蠕形螨感染相关眼表疾病的研究提供参考。
Demodex mites are the most common parasites found on human skin. They primarily reside in hair follicles, meibomian glands, and sebaceous glands of the eyes, and can trigger eye-related symptoms such as dry eyes, itchy eyes, and a pronounced foreign-body sensation. At present, it has been established that two types of Demodex mites parasitic in humans: Demodex follicle mite and Demodex sebum mite. Both types can induce various ocular surface diseases, including blepharitis, meibomian gland dysfunction, corneal disease, pterygium and ocular rosacea. However, due to subjective and objective factors, such as similar disease symptoms, omission of examination and a lack of awareness, these diseases are easy to misdiagnosis and underdiagnosis. Demodex mites exhibit a highly degree of age-dependence and can be detectd in asymptomatic adults. Consequently, the pathogenicity of Demodex mites has been a subject of debate. Existing studies have shown that Demodex mites can cause diseases through direct injury, by inducing hypersensitivity reactions, and by acting as carriers for bacteria. Effective means are available for detecting Demodex mite infections. Treatement options include warm compresses, eyelid cleaning, and the use of topical or systemic anti-mite drugs. Understanding the significance of Demodex mites in ocular diseases is crucial for accurate diagnosis and the formulation of appropriate management strategies. In recent years, there has been a growing body of research on Demodex mite. It is necessary to update the diagnositic and therapeutic techniques  for ocular surface diseases associated with Demodex mite infection. Therefore, this paper reviews the etiology, epidemiology, pathogenic mechanism, and detection methods of Demodex mite. It also discusses the relationship between Demodex mite infection and various ocular surface diseases, as well as the corresponding treatment methods, with the aim of providing a reference for the research on ocular surface diseases related to Demodex mite infections.

眼表是一个复杂且精细的系统,包括角膜、泪膜和结膜等多个结构。作为眼部直接与外界接触的部分,眼表不仅负责起着保护眼睛的作用,还在视觉功能中扮演着至关重要的角色。眼表微环境(ocular surface microenvironment,OSM)的平衡对眼睛的健康至关重要,任何微环境的失衡都可能引发多种眼表疾病,如干眼症、角膜炎等。随着现代人群生活方式的改变,视屏终端综合征(Visual Display Terminal,VDT)发生率急剧上升,破坏了OSM的平衡,导致眼表疾病的发病率大大升高,特别是干眼症的发病率显著增加,严重影响了患者的生活质量,因此,深入研究维持OSM稳态的调节机制以及探索有效的治疗策略显得尤为紧迫。近年来,研究突破了传统的解剖学框架,揭示了OSM的多系统协同调控机制,为维持眼表微环境稳态提供了新的思路。神经、淋巴、免疫系统之间相互调节,不仅有助于深入挖掘眼表疾病的关键分子和信号通路,还为发现新的生物标志物以及潜在治疗靶点提供了新的方向。因此,本文章系统综述了近年来OSM调节机制的最新进展,重点讨论神经、淋巴和免疫调节在维持OSM稳态中的作用,并分析三者之间的相互影响,以期为眼表疾病的预防及诊疗提供新的思路。
The ocular surface is a complex and sensitive system composed of includes various structures including the cornea, tear film and conjunctiva. As the part of the eye directly exposed to the external environment, the ocular surface not only protects the eye, but also plays a vital role in vision. The balance of the ocular surface(OSM) is essential for eye health, and any disruption can lead to ocular surface diseases, such as dry eye and keratitis. With the evolving lifestyle of the modern population, the incidence of Visual Display Terminal (VDT) syndrome has significantly increased, disturbing the OSM balance and leading to a rise in ocular surface diseases, particularly dry eye disease, which severely impacts patients' quality of life. Therefore, it is crucial to conduct comprehensive research into the regulatory mechanisms that maintain OSM homeostasis and explore effective therapeutic strategies. Recent studies have moved beyond traditional anatomical frameworks, uncovering the multi-system coordinated regulatory mechanisms of OSM, providing new insights into maintaining ocular surface homeostasis. The interplay between the nervous, lymphatic, and immune systems not only helps identify key molecules and signaling pathways involved in ocular surface diseases but also offers new opportunities for discovering novel biomarkers and potential therapeutic targets. This article presents a systematic review of the latest advancements in OSM regulatory mechanisms, focusing on the roles of nervous, lymphatic, and immune regulation in maintaining OSM homeostasis and analyzing the interactions among these systems, with the goal of offering new insights for the prevention and treatment of ocular surface diseases.

胬肉切除联合自体角膜缘干细胞移植治疗翼状胬肉的远期效果及眼表情况

Long-term Efficacy and Ocular Surface of Pterygium Excision Combined with Autologous Corneal Limbal Stem Cell Transplantation in Treatment of Pterygium

目的:探讨胬肉切除联合自体角膜缘干细胞移植治疗翼状胬肉的远期效果。
方法:2003年12月-2008年12月在我院就诊的原发性翼状胬肉患者55例57眼,均采用胬肉切除联合自体角膜缘干细胞移植治疗,术后随访2~8年,平均5.3年。观察创面愈合情况、治疗效果和并发症情况。
结果:术后早期患者均有不同程度的充血、水肿和异物感,3~5d 后症状逐渐减轻,一周左右完全消失。角膜创面完全上皮化时间为4~7d,平均4.83d。术后有持续性眼表刺激症状者7例,术后残留纤维组织者1例,角膜浅层瘢痕者4例。术后随访2~8年,平均5.3年,复发8例8眼 (复发率13.11%)。术后1年时复查OSDI 评分,所有患者较术前评分均有不同程度的降低,与术前相比,两组评分和构成比差异有统计学意义(P<0.05)。术后5年时复查OSDI评分较1年前稍有降低,但差异没有统计学意义(P>0.05)。术后1、5年时泪液基础分泌[(12.95±4.03) mm,(12.68±4.77) mm]和泪膜破裂时间 [(10.55±4.13)s, (10.03±4.64) s] 较术前[(9.48±3.34)mm,(8.14±3.57)s] 明显改善,差异有统计学意义(P<0.05)。
结论:应用胬肉切除联合自体角膜缘干细胞移植治疗翼状胬肉,术后早期症状轻、消失快,远期复发少,效果良好。
Purpose: To evaluate the long-term efficacy of pterygium excision combined with autologous corneal limbal stem cell transplantation in the treatment of pterygium.
Methods: Fifty five patients (57 eyes) diagnosed with primary pterygium undergoing pterygium excision combined with autologous corneal limbal stem cell transplantation between December 2003 and December 2008 were enrolled in this study. Postoperative follow-up endured for 2-8 years, 5.3 years on average. The wound healing, clinical efficacy and postoperative complications were observed.
Results: During early stage after surgery, all patients presented with varying degree of hyperemia, edema and sense of foreign body, which were gradually mitigated 3-5 d later and completely recovered in approximately 1 week. The corneal wound was completely epithelized within 4-7 d, 4.83 d on average. Seven patients had persistent ocular surface irritation symptoms after surgery, one case had residual fibrous tissues and four patients developed superficial corneal scars. Postoperative follow-up endured for 2-8 years, 5.3 years on average. Eight patients (8 eyes) recurred with a recurrence rate of 13.11%. In all patients,  postoperative OSDI score was significantly decreased at 1-year reexamination. OSDI score and constitution ratio significantly differed between two groups (both P<0.05).At postoperative 5 years, OSDI score was slightly lower compared with that in last year with no statistical significance(P>0.05).At post operative 1- and 5-year, basic tear secretion was (12.95 ±4.03) mm and (12.68 ±4.77) mm, tear film break-up time was (10.55±4.13)s and (10.03±4.64)s. Both indexes were significantly improved compared with (9.48±3.34) mm and (8.14±3.57)s before surgery (all P<0.05).
Conclusion: Pterygium excision combined with autologous corneal limbal stem cell transplantation is an efficacious treatment of pterygium with mild postoperative symptoms, rapid recovery, low long-term recurrence rate and high efficacy.

眼球表面鳞状细胞性肿瘤是比较常见的眼表肿瘤,包括鳞状细胞乳头状瘤、日光性角化病、结膜上皮内瘤变和鳞状细胞癌。深入了解这些病变的临床和病理学特点,有利于提高临床诊断的正确性。本文对眼球表面鳞状细胞性肿瘤的类型、临床表现和病理学特点进行阐述,以期对临床诊治提供参考。
Ocular surface squamous neoplasia (OSSN) is the most common tumor of the ocular surface, including squamous cell papilloma, actinic keratosis, conjunctival intraepithelial neoplasia and squamous cell carcinoma. The extended understanding of clinicopathological features could help improve clinical accurate diagnosis of these lesions. This article reviews the classification, clinical features and pathological characteristics of OSSN to provide the reference for clinical diagnosis and treatment.
