Revisiting the upper eyelid blepharoplasty advancement flap: adaptation for the repair of full-thickness upper eyelid defects—a case series

A blepharoplasty flap has been previously reported as a useful reconstruction approach for anterior lamellar defects lying between the lash line and the eyelid crease. We herein describe a variation of the blepharoplasty flap and suggest its use as an adjunct in the reconstruction of full-thickness lateral upper eyelid defects. Technique description and retrospective interventional case series. The reconstruction technique was used by an experienced oculoplastics surgeon (ASL) in 3 adults with malignant lesions involving the lateral upper eyelid margin, resulting in a post-excision 50% full-thickness defect between November 2017 and June 2020. The posterior lamella was reconstructed using an ipsilateral free tarsal graft and an inferiorly hinged transposition periosteal flap. The anterior lamella reconstruction was then performed using a local advancement flap utilizing the principles of upper blepharoplasty and Burow’s triangle. Almost full eyelid excursion and full gentle closure were evident at 1–2 weeks follow-up in all three cases. One case later developed 1–2 mm of gentle closure lagophthalmos and was managed successfully with topical lubricants. In all patients, the final eyelid contour and symmetry were adequate, with only minimal scarring, evident already 3 to 4 months postoperative. There were no major complications or need for revisions. The technique described herein highlights the utility of the blepharoplasty flap for lateral, full-thickness upper eyelid defects. This logical variation enables the reconstruction of significant defects using only local tissue, obeying the “like with like” principle, and helps avoid the need for a bridging flap. We provide preliminary evidence of the potential of a good cosmetic outcome of upper lid appearance and contour, together with a fast recovery of appropriate eyelid function.

Analysis of status and influencing factors of humanistic caring ability of ophthalmic nurses under ambulatory surgery code

Objective: To explore the current status of humanistic caring ability of ophthalmic nurses under ambulatory surgery and its influencing factors. Methods: Totally 231 ophthalmic nurses in departments of ambulatory surgery in an ophthalmic center in Guangdong province were investigated by general information scale, Caring Ability Inventory (CAI), Nurses’ Job Satisfaction Scale, the Jefferson Scale of Empathy-Health Professionals and Narrative Competence Scale. Results: The overall rate of CAI was 75.59%. Clinical leadership, nurses’ job satisfaction, empathy and narrative competence were the influencing factors of the humanistic caring ability (P<0.05). And those factors explained 61.7% of the variability of ophthalmic nurses' humanistic caring ability. Conclusion: The humanistic caring ability of ophthalmic nurses under ambulatory surgery code was at the medium level. Nursing managers should take various measures to improve the job satisfaction of nurses, strengthen the training of empathy and narrative competence, so as to improve the humanistic care ability of nurses.

The correlation between depression level and clinical indicators of dry eye

Objective: This study aims to explore the correlation between clinical signs and depression levels in patients with dry eye disease, providing a reference for the construction of personalized treatment for dry eye patients. Methods: A total of 232 dry eye patients admitted to the outpatient department of the Affiliated Hospital of Yanbian University from 2021-August to 2023-August were selected for investigation. OSDI scores, basic tear secretion test (SIt), tear film rupture time (BUT), corneal fluorescin staining (FL) scores were collected. Self-rating Depression Scale (SDS) was used to evaluate the depression degree of the patients and they were divided into four groups: no depression, mild depression, moderate depression and severe depression. Spearman correlation analysis was used to analyze whether there were differences in clinical indicators of dry eye patients with different degrees of depression, whether there was a gap between dry eye indicators and SDS scores in male and female patients, and whether there was a gap between dry eye indicators and SDS scores in different occupational groups. Results: The total SDS score of dry eye patients was (53.80±15.18). Spearman correlation analysis showed that the total SDS score was positively correlated with OSDI and FL (P<0.001), and negatively correlated with SIt and BUT (P<0.001). There were significant differences in clinical indexes among the four groups (P<0.05). Conclusion: Dry eye patients generally have a high level of depression, which is significantly correlated with their condition. In addition, there are also differences in clinical indicators of dry eyes among different occupations and genders. Keywords: Dry Eye; Depression; Self-Rating Depression Scale; Correlation

The research on therapeutic control of myopia

Myopia is a hot topic that has attracted more and more attention in recent years, and the trend of younger age and high incidence of myopia has seriously threatened the daily life of patients and become a social problem related to people's health and even the national economy and people's livelihood. Because its pathogenesis is not fully clarified, it is difficult to block its progression from the etiological point of view. Therefore, this article aims to discuss some therapeutic control methods of myopia to reduce the incidence of myopia and improve the quality of life of patients.

Past, present and future of suspensory IOL implantation

In recent years, ectopia lentis has gradually attracted the attention of ophthalmologists, and the exploration and improvement of surgery for ectopia lentis has been in progress. With the invention of phacoemulsification technology and the improvement of intraocular implant materials, the implantation and fixation of intraocular lens (IOL) in patients with ectopia lentis has become the focus of ophthalmologists. Suspensory IOL implantation is a classic operation for IOL implantation in patients with severe lens dislocation or aphakia with insufficient capsular bag support. It has mainly gone through three stages: traditional trans-sclera suture-fixed posterior chamber IOL, kontless posterior chamber IOL fixation represented by Z suture, and sutureless intrascleral IOL fixation. Understanding and mastering the advantages and limitations of various IOL fixation methods is of great importance for improving the diagnosis and treatment level of ophthalmologists and as well as the prognosis of patients.

Consistency Analysis of Corneal Curvature Measurement by Five Instruments

The five instruments, including OPD-SCAN III wavefront aberrometer, Pentacam AXL anterior segment analyzer, IOLMaster 700, Lenstar LS900, and NIDEK ARK-1 automatic computerized refractor, were applied to measure the corneal curvature, corneal astigmatism, and axis alignment of cataract patients before surgery. The consistency of the measurements was analyzed and evaluated to guide clinical application and improve the accuracy of refractive cataract surgery. A total of 52 patients (103 eyes) who underwent phacoemulsification combined with intraocular lens implantation surgery from December 2019 to March 2020 at the Fourth Affiliated Hospital of China Medical University were selected. The five instruments were used to measure the flat corneal curvature (K1), steep corneal curvature (K2), and the mean keratometry (Km) of the eyes being examined. OPD-SCAN III was used as the commonly used instrument, and the results were compared pairwise with the other four instruments to explore whether there were statistical differences. The consistency analysis was performed using the Bland-Altman test. The results showed that there was a statistically significant difference between OPD-SCAN III and Pentacam AXL (P<0.05), while there was no statistically significant difference between OPD-SCAN III and the other three instruments. In conclusion, the consistency of OPD-SCAN III wavefront aberrometer with IOLMaster 700, Lenstar LS900, and NIDEK ARK-1 automatic computerized refractor was good, but there were differences with the measurements made by Pentacam AXL anterior segment analyzer.

Retinal changes in prediabetes and pre-clinic stage of Diabetes retinopathy

Diabetes retinopathy (DR) is the main cause of visual impairment in the working population worldwide. Patients with prediabetes and pre-clinic diabetic retinopathy are at high risk of developing diabetic retinopathy. At this stage, morphological and functional changes of retinal neurons as well as changes of retinal microvessels and small blood vessels were found. The changes of retinal nerve structure can be partly reflected by observing the thickness of retina and nerve fiber layer. Changes in color vision, contrast sensitivity, visual field and electrophysiology reflect changes in retinal nerve function. Due to the development of optical coherence tomography angiography, we can also observe changes in retinal microvessels prior to clinical detection of DR. In addition, many biomarker can also predict and evaluate DR. Since there is no way to prevent the occurrence and progress of DR at present, we can pay more attention to DR at an early stage by observing the above retinal changes to reduce its prevalence and minimize the visual damage caused by DR.

Therapeatic effect evaluation of conjunctival cyst extraction under slit lamp microscope

Objective To investigate the safety ,feasibility and effectiveness of conjunctival cyst extraction under slit lamp microscope .Methods:57 eyes from 56 patients with conjunctival cyst were treated by using a disposable 1 ml syringe to aspirate the fluid in the capsule and complete extraction of the capsule with negative pressure.Postoperative follow-up was conducted to observe the ocular surface disease index ,Results Conjunctival cysts were removed completely in 57eyes from 56patients,and no complications occurred after the operation.No recurrence was observed for 2years .Conducsion: Removing conjunctival cyst by using disposable 1ml syringe under slit lamp microscope is safe ,feasible and effective.The patients have good medical experience.
