随着微创玻璃体切除术(pars plana vitrectomy,PPV)的广泛开展和手术技术的提高,患者对手术后视觉质量的要求越来越高。白内障是PPV术后最常见并发症,而具有玻璃体切除史的白内障患者屈光变异大,预测难度高。本文综述了生物测量误差、人工晶状体屈光力计算公式选择以及有效晶状体位置预测等影响有玻璃体切除手术史的白内障患者术后屈光误差的主要因素,旨在为降低这一类特殊人群白内障术后屈光误差提供参考。
With the widespread application of minimally invasive vitrectomy and the improvement of surgical techniques, the demands of patients for better postoperative visual quality are increasing. Cataract is the most common complication after vitrectomy, whereas the refractive outcomes of cataract patients with prior vitrectomy are viable and difficult to predict. In this paper, the main factors affecting postoperative refractive error of cataract patients with a history of vitrectomy, such as biometric error, selection of intraocular lens calculation formulas and prediction of effective lens position, were reviewed in order to provide reference for reducing postoperative refractive error of this special group of cataract patients.

角膜因其特有的内部有序排列结构具有高度透明性,是眼前部重要的屈光间质,又因其缺乏血液供应,角膜疾病恢复周期较长、病情易反复出现,严重影响患者的视力和日常生活。基质金属蛋白酶(matrix metalloproteinase, MMP)通过参与组织蛋白降解重塑、血管生成、炎症免疫反应等,在肺纤维化、动脉粥样硬化等众多疾病中具有重要作用,其中MMP-2(明胶酶A)在眼部相关疾病中的作用也逐渐受到关注。单纯疱疹病毒性角膜炎具有高度致盲性,反复发作会导致角膜失去透明性,角膜混浊逐渐加重,最终导致失明。研究者通过对MMP-2在HSK、角膜溃疡中的病理作用机制进行分析研究,发现MMP-2的特异性抑制药物在角膜炎中具有一定的临床应用前景,未来或可从本文的研究角度出发,适当增加相关的药物治疗研发,解决目前治疗的局限性,以期为角膜疾病患者带来新的有效治疗方案。
The cornea has a high degree of transparency due to its unique internal orderly arrangement structure, and as a major refractive media in the anterior eye. However, due to its lack of blood supply, the recovery period of corneal diseases is long and the condition is prone to recurrence, corneal diseases seriously affecting the patient's vision and daily life. Matrix Metalloproteinases play an important role in many diseases, such as pulmonary fibrosis and atherosclerosis, by participating in tissue protein degradation and remodelling, angiogenesis, inflammatory immune response, etc. Among them, the role of MMP-2 in ocular diseases has also been gradually explored and studied. Herpes simplex virus keratitis is highly blinding, and repeated attacks can cause the cornea to lose transparency, gradually worsen corneal opacity, ultimately resulting in blindness. By analysing the mechanism of MMP-2 in herpesvirus keratitis and corneal ulcers, researchers were found that specific inhibitions of MMP-2 have certain clinical application prospects in keratitis. In the future, from the perspective of this study, it may be appropriate to increase the research and development of related drug treatments, solve the limitations of current treatments, and bring new effective treatment options for corneal disease patients.

全球范围内,肥胖或超重问题持续加剧,据《2024年世界肥胖报告》及近期数据,预计2035年受超重或肥胖影响的成年人将达33亿,我国成人肥胖或超重患病率已超50%且呈上升趋势。肥胖会增加2型糖尿病、心血管疾病等多种疾病风险,减轻5%~15%体重可改善相关并发症,因此减重关注度日益提升,除运动和饮食控制外,减重药物与手术等手段不断涌现。然而减重药物在减轻体质量(体重)的同时,其潜在的危害不容忽视,有研究表明减重药物可对眼部产生不良影响:导致眼压升高使患者出现眼睛疼痛、视力模糊的症状;影响眼部血液循环和营养供应,发生炎症感染;出现眼睛疲劳干涩,对糖尿病性视网膜病变产生影响等。本文聚焦目前主流的三类减重药物——非中枢性减重药物(以脂肪酶抑制剂为代表)、中枢性减重药物(含拟儿茶酚胺类制剂和5-羟色胺受体激动剂)、兼有减重效果的降糖药物(包括GLP-1RA、GLP-1/GIP双受体激动剂、二甲双胍等),详细论述其应用及对眼部的不良反应。同时,也提及部分药物可能对眼部产生的益处,如二甲双胍、GLP-1RA或对青光眼、糖尿病视网膜病变有潜在保护作用。本文旨在为用药人群、相关医护及从业人员提供理论指导,强调使用减重药物时需警惕眼部风险,平衡疗效与安全性。
Globally, the problem of obesity or overweight continues to intensify. According to the "2024 World Obesity Report" and recent data, it is estimated that by 2035, the number of adults affected by overweight or obesity will reach 3.3 billion. In China, the prevalence of obesity or overweight among adults has exceeded 50% and is on the rise. Obesity increases the risk of various diseases such as type 2 diabetes and cardiovascular diseases. Reducing body weight by 5% to 15% can improve related complications. Therefore, the focus on weight loss is increasing. Besides exercise and dietary control, weight loss drugs and surgeries are constantly emerging. However, while weight loss drugs can reduce body weight, their potential harms should not be ignored. Studies have shown that weight loss drugs can have adverse effects on the eyes: causing increased intraocular pressure, leading to symptoms such as eye pain and blurred vision; affecting blood circulation and nutrient supply to the eyes, resulting in inflammation and infection; causing eye fatigue and dryness, and having an impact on diabetic retinopathy, etc. This article focuses on the three main types of weight loss drugs currently available - non-central weight loss drugs (represented by fat enzyme inhibitors), central weight loss drugs (including catecholamine-like preparations and 5-hydroxytryptamine receptor agonists), and hypoglycemic drugs with weight loss effects (including GLP-1RA, GLP-1/GIP dual receptor agonists, metformin, etc.), and elaborates on their applications and adverse reactions to the eyes. At the same time, it also mentions the potential benefits that some drugs may have on the eyes, such as metformin and GLP-1RA, which may have a protective effect on glaucoma and diabetic retinopathy. This article aims to provide theoretical guidance for drug users, related medical staff, and professionals, emphasizing the need to be vigilant about eye risks when using weight loss drugs and to balance efficacy and safety.

Strengthening medical humanities education is of vital significance in the standardized training of ophthalmology residents, as it contributes to enhancing their doctor-patient communication skills and professional ethics. This paper examines the current situation and significance of humanities education for ophthalmology residents. On this basis, the paper suggests five concrete methods and approaches for fostering residents' humanistic qualities, such as: (1) strengthening doctor-patient communication awareness through pre-training humanities courses to establish a proper humanistic mindset before clinical practice; (2) developing empathy by participating in hospital service center activities through hands-on patient service experiences; (3) deepening humanistic sentiment and enhancing professional ethics through medical humanities reading activities featuring classical works; (4) discussing clinical ethical dilemmas via departmental humanities analysis meetings to help residents develop decision-making skills in complex scenarios; (5) expanding humanistic horizons through the integration of ideological and political education with medical humanities practice, ensuring professional skill development aligns with humanistic principles. This paper aims to offer theoretical support and practical guidance for the humanities education of ophthalmology residents, with the objective of cultivating more ophthalmology professionals with both clinical excellence and high humanistic caliber.

基于医院小儿眼科常见病种分析儿童眼科药物临床试验现状

目的:结合医院小儿眼科常见病种调研国内儿童眼用制剂药物临床试验开展现状,为儿童眼用制剂药物临床试验的发展提供参考。方法:基于国家药物临床试验登记与信息公示平台和医院信息系统,结合郑州市第二人民医院儿童眼科病种,分析儿童眼科药物临床试验开展情况。 结果:医院门诊就诊患者以屈光不正最多(占68.49% ),其次是结膜炎(占11.25%),再次是斜视(占8.60%)。平台共检索到相关临床试验165项,儿科专用药物临床试验25项,其中延缓青少年近视方面试验24个,小儿结膜炎试验1个,其他病种药物临床试验检测结果为0;药物临床试验中以I期和Ⅲ期临床试验为主;延缓青少年近视方面试验已完成试验8项,其他都在进行中。结论:儿童眼科疾病的药物治疗尚有巨大的临床需求未满足,直接关系到了儿童健康权益保障与生命安全维护。当前国内儿童眼用制剂研发呈现企业参与度低、创新动力不足的现状,其核心制约因素在于项目少、难度大、涉及更多的伦理问题。建议通过政产学研协同创新,系统性推进儿童眼用制剂的研发进展,切实解决我国儿童眼病治疗用药难的问题,为儿童视觉健康提供有力保障。

Objective:Combined with the common diseases of pediatric ophthalmology in our hospital, the current situation of clinical trials of ophthalmic preparations for children in China was investigated to provide reference for the development of clinical trials of ophthalmic preparations for children in China. Methods:Based on the national drug clinical trial registration and information publicity platform and hospital information system, combined with the pediatric ophthalmic diseases in our hospital, the development of pediatric ophthalmic drug clinical trials was analyzed. Results:Among the outpatients in our hospital, ametropia was the most common ( 68.49 % ), followed by conjunctivitis ( 11.25 % ) and strabismus ( 8.60 % ). A total of 165 clinical trials and 25 clinical trials of pediatric drugs were retrieved from the platform, including 24 trials on delaying juvenile myopia, 1 trial on pediatric conjunctivitis, and 0 clinical trial on other diseases. Drug clinical trials were mainly phase I and phase III clinical trials. Eight trials have been completed to delay juvenile myopia, and others are in progress. Conclusions:There is still a huge clinical demand for the drug treatment of children 's eye diseases, which is directly related to the protection of children 's health rights and life safety. At present, the research and development of children 's ophthalmic preparations in China presents the current situation of low enterprise participation and insufficient innovation motivation. The core constraints are fewer projects, greater difficulty, and more ethical issues. It is suggested to systematically promote the research and development of ophthalmic preparations for children through the collaborative innovation of government, industry, university and research, so as to effectively solve the problem of difficult medication for children 's eye diseases in China and provide a strong guarantee for children 's visual health.

近年来,热疗因其操作简便和显著疗效,在干眼及相关眼病治疗中受到关注。热疗通过加热睑板腺区域,软化睑脂,改善睑板腺导管通畅,广泛应用于睑板腺囊肿、睑腺炎、睑缘炎及睑板腺功能障碍等眼病。此外,热疗还有效缓解视疲劳和干眼不适,促进眼部创口愈合,减少感染风险。在中国古代,《黄帝内经》及《韩非子》早期记载了热疗技术的应用。西汉《淮南子》提到烧灼法用于眼病,东晋葛洪的《肘后备急方》描述了热敷治疗眼部不适的方法。隋唐时期的《备急千金要方》和《外台秘要》详细介绍了热疗的应用,宋代《圣济总录》和《龙树菩萨眼论》进一步精细化了热疗技术。明清时期的《普济方》和《目经大成》中记载了热疗的新进展。由此,古代热疗在眼科应用中积累了丰富的经验,尽管现代医学技术不断进步,但这些传统方法仍具有重要参考价值。
In recent years, heat therapy has gained attention for its simplicity and significant efficacy in treating dry eye and related ocular conditions. By heating the meibomian gland area, heat therapy softens meibomian secretions and improves gland duct patency, making it widely used for chalazion, hordeolum, blepharitis, and meibomian gland dysfunction. Additionally, it effectively alleviates visual fatigue and dry eye discomfort, promotes ocular healing, and reduces the risk of infection.In ancient China, the application of heat therapy was recorded early in texts such as the Huangdi Neijing and Han Feizi. The Huainanzi from the Western Han Dynasty mentions the use of cautery for eye diseases, while Zhouhou Beiji Fang by Ge Hong in the Eastern Jin Dynasty describes methods for treating eye discomfort with hot compresses. During the Sui and Tang dynasties, Beiji Qianjin Yaofang and Wai Tai Mi Yao provided detailed accounts of heat therapy applications. The Song Dynasty works *Shengji Zonglu* and Longshu Bodhisattva's Eye Treatise further refined the techniques. The Ming and Qing dynasties’ texts Pujifang and Mujing Dacheng documented new advancements in heat therapy.In summary, ancient heat therapy for ophthalmology accumulated substantial experience. Despite advances in modern medical technology, these traditional methods still hold significant reference value.

Retinal vein occlusion (RVO) is one of the leading retinal diseases causing vision impairment and is often associated with retinal ischemia, hemorrhage, fluid leakage, and macular edema, ultimately resulting in decreased vision or even permanent vision loss. Currently, the primary treatment for RVO-associated macular edema is intravitreal injection of anti-vascular endothelial growth factor (VEGF) agents. However, the pathological mechanisms of RVO are not limited to VEGF alone, but also involve angiopoietin-2 (Ang-2). Under pathological conditions, Ang-2 disrupts vascular stability, induces neovascularization, and exacerbates inflammatory responses, thereby accelerating the progression of RVO. Faricimab, as a bispecific antibody, can simultaneously inhibit both VEGF-A and Ang-2 pathways, which are critical in RVO pathogenesis, and has shown potential advantages in improving visual outcomes. The article provides a detailed discussion on the mechanism of action, clinical applications, comparison with related therapeutic agents, and future development prospects of Faricimab in the treatment of RVO, offering a theoretical basis and reference for its further application in ophthalmology.

广角 SS-OCT En Face 结构投射图揭示未成年人玻璃体早期液化特征

目的:应用广角扫频源光学相干断层扫描成像(swept-source optical coherence tomography, SS-OCT)的en face结构投射图研究玻璃体早期液化特征。方法:使用SS-OCT进行18 mm×18 mm 的容积(Cube)扫描,创建并分析健康未成年人(年龄5~18岁)70眼的系列玻璃体en face结构投射图。 结果:在未成年人中,视网膜前的玻璃体包含4种液化结构,分别为后皮质前玻璃体囊袋(posterior precortical vitreous pocket, PPVP)、视盘前Martegiani区(the area of Martegiani, AM)、血管前液化裂隙(prevascular vitreous fissures,PVF)和液化池(cistern)。所有研究眼均能检出PPVP、AM和PVF,其中22眼(31.4%)的PPVP和AM连通。41眼(58.6%)可检出液化池,且其年龄大于未检出液化池的个体(=0.01),液化池的发生与年龄呈正相关(rs=0.315,=0.008)。液化池的象限空间分布频率依次为颞上(90.2%)、鼻上(58.5%)、颞下(36.6%)、鼻下(24.4%),最常累及颞上象限(P<0.001)。 结论:PPVP、AM和PVF是健康人群视网膜前玻璃体早期液化过程中均出现的特征。液化池的发生与年龄呈正相关,最常出现在颞上象限,可能是年龄相关性玻璃体液化变性的结果。
Objective: To investigate the early vitreous liquefaction characteristics using en face structural projection images obtained by wide-angle swept-source optical coherence tomography (SS-OCT). Methods: SS-OCT was employed to perform 18*18mm volumetric (Cube) scans. A series of en face structural projection images of the vitreous were created and analyzed for 70 eyes from healthy minors aged between 5 and 18 years. Results: In minors, four types of vitreous liquefaction structures were identified anterior to the retina: pre-posterior vitreous pocket (PPVP), the preoptic area of Martegiani (AM), pre-vascular liquefaction fissures (PVF), and cisterns. PPVP, AM, and PVF were detectable in all studied eyes, with PPVP and AM being interconnected in 22 eyes (31.4%). Cisterns were observed in 41 eyes (58.6%), and the mean age of individuals with cisterns was higher than those without (=0.01). The occurrence of cisterns positively correlated with age (r=0.315; P=0.008). The frequency of cistern quadrant distribution was highest in the superotemporal quadrant (90.2%), followed by the superonasal quadrant (58.5%), inferotemporal quadrant (36.6%), and inferonasal quadrant (24.4%). The superotemporal quadrant was the most frequently affected (<0.001). Conclusion: PPVP, AM, and PVF are features consistently observed in the early vitreous liquefaction process anterior to the retina in healthy individuals. The occurrence of cisterns positively correlates with age and is most common in the superotemporal quadrant, possibly representing the result of age-related vitreous liquefaction degeneration. These findings provide a theoretical foundation for studying the pathogenesis of vitreoretinal interface diseases.

腰椎感染致双眼内源性眼内炎一例及文献回顾

内源性真菌性眼内炎(endogenous fungal endophtalmitis, EFE)是最具破坏性的眼部感染之一,在临床上较少见。如诊断和治疗不及时,可严重损害患者视力,甚至需摘除眼球。由于EFE发病隐匿,病程较长,病原学涂片和培养阳性率较低,早期临床症状与葡萄膜炎相似,极易被误诊和漏诊,延误治疗时机。EFE最常见的感染灶来源为肝脏、肺、尿路、脑膜炎、胃肠道、心内膜以及骨髓。文章报道了一例腰椎感染致双眼内源性念珠菌性眼内炎的男性患者,66岁,因“右眼视力下降1周”首诊于眼科,专科检查见右眼玻璃体炎性混浊,初诊为右眼葡萄膜炎,予抗炎等治疗症状无好转,右眼视力持续下降,右眼前房穿刺抽液送检提示:热带念珠菌感染,之后左眼视力也逐渐下降,加之患者近期于骨科住院,术中腰椎间盘退变的纤维软骨组织DNA-病原微生物宏基因组检测结果示热带念珠菌,考虑双眼EFE,予全身及局部使用抗真菌药物联合双眼玻璃体切割手术,治疗后患者视力恢复良好,随访1年无复发。该病例及相关文献回顾,有助于加深临床医生对此类疾病的认识,为今后临床诊疗提供一定思路,也起到一定警示作用。
Fungal endophthalmitis is one of the most destructive eye infections and is relatively rare in clinical practice.If not diagnosed and treated promptly, it can severely damage vision and even lead to enucleation.Due to its insidious onset, long course, low positive rates in smears and cultures, and early clinical symptoms similar to uveitis, it is prone to misdiagnosis and missed diagnosis, leading to delayed treatment. A review of the literature indicates that the most common sources of EFE infection are the liver, lung, urinary tract, meningitis, gastrointestinal tract, endocarditis and osteomyelitis.In this paper, we report a case of lumbar spine infection causing bilateral candidal endophthalmitis in a 66-year-old male patient.He initially presented to the ophthalmology department of our hospital with a one-week history of decreased vision in the right eye, specialized examination revealed inflammatory opacity in the vitreous of the right eye, initially diagnosed as uveitis and treated with anti-inflammatory therapy without improvement.As the vision in the right eye continued to decline, aqueous humor aspiration from the anterior chamber of the right eye indicated infection with tropical Candida.Subsequently, the vision in the left eye also gradually decreased.Considering the recent hospitalization in the orthopedic department for lumbar disc degeneration, metagenomics analysis of fibrous cartilage tissue DNA during surgery detected tropical Candida, suggesting bilateral endogenous fungal endophthalmitis,The patient was treated with systemic and local antifungal medications in combination with bilateral vitrectomy surgery.After treatment, the vision recovered well, and there was no recurrence during a one-year follow-up.The objective of this thesis is to deepen the understanding of clinicians on this type of disease by reporting this case and reviewing relevant literature, providing some insights for future clinical diagnosis and treatment, and serving as a warning.

先天性晶状体脱位(congenital ectopia lentis, CEL)是一种罕见的遗传相关性疾病,其主要临床特征是晶状体悬韧带先天性发育异常,导致晶状体偏离正常解剖位置。随着病情的进展,CEL可引起高度屈光不正甚至弱视外,还可能导致继发性青光眼和视网膜脱离等严重的并发症。目前,手术仍是改善CEL患儿视觉质量及防治并发症的主要手段。常用的手术方式包括晶状体摘除术、前房型人工晶状体(intraocular lens, IOL)植入术、囊袋支撑装置联合IOL植入术及经巩膜IOL固定术等,这些手术方式各具特点,但目前最佳手术方式仍未有定论。既往大量文献表明,手术能够显著改善CEL患儿视力,但随着眼球的生长发育,CEL患儿术后屈光状态常出现近视漂移。此外,术后并发症如缝线暴露,IOL瞳孔夹持、IOL脱位、视网膜脱离等仍有可能发生,需要长期的严密随访。这些因素都使得CEL的治疗具有挑战性。为此,文章就CEL的手术方式、视力预后、术后屈光变化及术后并发症进行综述,旨在为该疾病的临床诊断及治疗提供更为全面和深入的理解。

Congenital ectopia lentis (CEL) is a rare genetic disorder characterized by the displacement of the lens from its normal anatomical position due to abnormalities in the lens zonular. As the progression of the disease, CEL can lead to high refractive error, even amblyopia, as well as other serious complications such as secondary glaucoma and retinal detachment. Currently, surgical intervention remains the primary method to improve the visual quality and prevent complications in children with CEL.Common surgical options include lens extraction, anterior chamber intraocular lens (IOL) implantation, IOL implantation combined with capsular tension devices, and transcleral fixation of IOL. Each surgical approach has its own characteristics, but there is currently no consensus on the best surgical method. Previous literature has shown that surgery can significantly improve vision in children with CEL; however, due to the growth of the eye, postoperative refractive status often experiences myopic shift. Additionally, complications such as suture exposure, IOL pupil capture, IOL dislocation, and retinal detachment may still occur, necessitating long-term close follow-up. These factors make the treatment of CEL challenging. This article reviews the surgical approaches, visual prognosis, postoperative refractive changes, and postoperative complications associated with CEL, aiming to provide a more comprehensive and in-depth understanding for the clinical diagnosis and treatment of this disease.
