目的:了解眼科住院医师规范化培训学员(规培生)的沟通技能态度和人际沟通能力现状。方法:对中山大学中山眼科中心三个年级规培生的沟通态度和人际沟通能力进行问卷调查,并进一步分析其沟通态度与沟通能力的相关性。结果:共纳入196名规培生,沟通积极态度量表总分为51.38±6.52,消极态度为36.80±5.70,人际沟通能力为2.41±0.40,辅导与咨询维度得分为2.50±0.64,提供有效的负面反馈维度得分为2.61±0.50,支持性沟通维度得分为2.29±0.44。沟通技能的积极态度与人际沟通能力和三维度之间显著相关(P<0.01)。人际沟通技能得分与性别、规培年级、喜欢眼科专业的程度、得到家庭的关爱程度、得到朋友的关爱程度等因素显著相关(P<0.05)。结论:眼科规培生对沟通技能的积极态度优秀,但是总体沟通能力较弱,并受到多种因素影响。应充分重视规培生人际沟通能力的培养,开展医患沟通的专业化培训。
Objective: To evaluate the current status and correlations between attitudes and competency for interpersonal communication in ophthalmology standardized training trainee. Methods: A questionnaire survey was conducted on the communication attitudes and interpersonal communication ability of standardized training trainees from three grades in Zhongshan Ophthalmic Center, affiliated to Sun Yat-sen University, and the correlation between communication attitude and communication ability was further analyzed. Results: A total of 196 trainees were included. The scores of positive attitude, negative attitude, and interpersonal communication skills were 51.38±6.52, 36.80±5.70, and 2.41±0.40, respectively. In addition, the dimension of coaching and counseling scores were 2.50±0.64, the dimension of providing effective and negative feedback scores were 2.61±0.50, and the dimension of supportive communication scores were 2.29±0.44. The positive attitudes of communication skills were significantly related to the interpersonal communication skills and three-dimensionality (P<0.01). The interpersonal communication skills were significantly associated with sex, grades, interest in ophthalmology, social support from the family and friends (P<0.05). Conclusion: Ophthalmology trainees have excellent positive attitudes toward communication skills, but the overall communication skills are weak and influenced by a variety of factors. More attention should be paid to the development of interpersonal communication skills of the trainees, and specialized training in doctor-patient communication is warranted.
近年来,眼科手术后的疼痛受到关注,部分患者术后表现出疼痛区域扩大,神经性畏光。术后这种 对伤害性刺激性信号反常性增加的现象称之为痛觉过敏。痛觉过敏的发展会导致患者延迟术后恢 复等问题。此外,痛觉过敏还会引起患者不适,诱使患者使用更多的止痛药而产生相关的不良反 应。而TRPV1/TRPA1是一种伤害性感受器,能够被伤害性刺激特异性激活而诱发痛觉过敏。尽管 如此,目前关于眼科术后的痛觉过敏发病机制未完全阐明,TRPV1如何增敏导致痛觉过敏的分子生 物学过程尚未明确。
The postoperative pain of ophthalmic surgery has been widely concerned in recent years. Some patients show enlarged painful areas and neurophotophobia postoperatively. This abnormal increase in noxious irritant signals after surgery is called hyperalgesia. The development of hyperalgesia can result in delayed postoperative recovery for patients. In addition, hyperalgesia can also cause discomfort for patients, and induce patients to use more analgesics, which can have related side effects. TRPV1/TRPA1 is a nociceptor, which can be specifically activated by nociceptive stimuli to induce hyperalgesia. Nevertheless, the pathogenesis of hyperalgesia after ophthalmology surgery has not been fully elucidated, and the molecular signal pathway of how TRPV1 sensitizes and causes hyperalgesia is not yet clear.
目的:探讨构建眼科高值手术耗材管理体系的思路和具体实践方法,加强眼科高值手术室耗材管理。方法:通过信息化手段,建立眼科高值手术耗材可视化管理系统,与医院各信息系统对接,整合相关数据,建立眼科高值手术耗材合理使用监测系统,将耗材的供应、使用到监测的全生命周期管理与日常监管相结合。结果:通过对眼科高值手术耗材全生命周期体系的建立,实现了术前可控制、术中可监测、术后可追溯的全流程管理。结论:该体系的建立可提高眼科手术高值耗材的管理效率及质量。
Objective: To construct an ideal and practical method for managing ophthalmic high-value surgical supplies to strengthen the management of ophthalmic operating room. Methods: A visual management system of ophthalmic high-value surgical supplies was established by using information technology connected with hospital information systems, then the data were integrated to construct a monitoring system for real-time status of ophthalmic high-value surgical supplies, including the whole life cycle management of the surgical supplies. Results: Through the establishment of the system for whole life cycle of high-value ophthalmic surgical supplies, the whole process management realized the supplies for preoperative control, intraoperative monitoring and postoperative trace. Conclusion: Constructing the system solves the daily management problem of ophthalmic high-value surgical supplies, reduces the blind area of management, and strengthens the supervision of surgical supplies in the ophthalmic operating room.
目的:对比喉罩与气管插管在小儿眼科全身麻醉中的应用情况,选取小儿全身麻醉的适宜气道管理方式。方法:选取2019年6月至2021年4月在天津市眼科医院进行眼科全身麻醉手术的86例患儿为研究对象,根据全身麻醉诱导后置入气管导管或喉罩分为插管组与喉罩组,每组43例。麻醉诱导后插入气管导管或喉罩后实施容量控制通气。通过回顾收集麻醉记录单查询麻醉诱导前(T0)、麻醉诱导后(T1)、插入气管导管或喉罩时(T2)、手术开始时(T3)、手术结束时(T4)、移除喉罩或气管导管时(T5)检测心率、平均动脉压,以及插管后5、10、20 min时的平均气道压和呼气末CO2分压。根据麻醉记录单查询每例患者气管插管或喉罩置入的次数、苏醒期呛咳及躁动的发生情况。比较2组麻醉时间、置管/喉罩首次成功率、拔管/撤罩时间、自主呼吸恢复时间、麻醉复苏时间。观察2组患者并发症(呛咳、躁动发生情况)的发生情况。结果:在T2、T3和T5时,插管组心率明显高于喉罩组,差异有统计学意义(P<0.05);在T2、T3、T4和T5时,插管组平均动脉压明显高于喉罩组,差异有统计学意义(P<0.05)。插管后5、10和20 min时,插管组平均气道压均高于喉罩组,差异有统计学意义(P<0.05)。2组不同时间点呼气末CO2分压的比较,差异无统计学意义(P<0.05)。喉罩组麻醉复苏期间呛咳和躁动的发生率明显低于插管组,差异有统计学意义(P<0.05)。结论:喉罩具有操作简单、对气道刺激较小等优点,用于小儿眼科全身麻醉时对血流动力学的影响较小,气道反应性低,并发症较少,值得在临床推广使用。
Objective: To select the appropriate airway management mode for children under general anesthesia by comparing the application of laryngeal mask and endotracheal intubation in pediatric ophthalmology. Methods: A total of 86 children who underwent ophthalmic general anesthesia surgery in our hospital in June 2019 and April 2021 were selected as the study subjects, and the tracheal catheter or laryngeal mask was placed after induction of general nesthesia, and the group of 43 cases were divided into intubation group and laryngeal mask group. Volumecontrolled ventilation is performed after induction of anesthesia after insertion of a tracheal catheter or laryngeal mask. Heart rate and mean arterial pressure were detected by retrospectively collecting anesthesia records alone before anesthesia induction (T0), after anesthesia induction (T1), when a tracheal catheter or laryngeal mask was inserted (T2), at the beginning of surgery (T3), at the end of surgery (T4), and when the laryngeal mask or tracheal catheter was removed (T5). As well as the average airway pressure and end-expiratory CO2 partial pressure at 5, 10, and 20 min after intubation. The number of endotracheal intubations or laryngeal hoods, the occurrence of cough during awakening, and agitation in each patient was queried according to the anesthesia record. The timing of anesthesia, the first success rate of catheterization/laryngeal mask, the time of extubation/uncommissioning, the time of spontaneous breathing recovery, and the time of anesthesia resuscitation were compared between the 2 groups. The occurrence of complications (choking cough, agitation) in both groups of patients was observed. Results: At T2, T3 and T5, the heart rate in the intubation group was significantly higher than that in the laryngeal mask group (P<0.05), and at T2, T3, T4 and T5, the mean arterial pressure in the intubation group was significantly higher than that in the laryngeal mask group(P<0.05). At 5, 10, and 20 min after intubation, the mean airway pressure in the intubation group was higher than that in the laryngeal mask group, and the difference was statistically significant (P<0.05). There was no statistical difference in the partial pressure of CO2 at the end of expiratory breath at different time points between the 2 groups (P<0.05). The incidence of choking cough and agitation during anesthesia resuscitation in the laryngeal mask group was significantly lower than that in the intubation group, and the difference was statistically significant (P<0.05). Conclusion: The laryngeal mask has the advantages of simple operation and less stimulation of the airway, and when used for pediatric ophthalmic general anesthesia, it has less hemodynamic effect, low airway responsiveness and fewer complications, and is worth promoting in clinical practice
小儿眼科麻醉的挑战是既要保障患儿的麻醉安全又要提高手术接台效率。麻醉前的充分评估和详细的准备可以有效地降低麻醉期间的并发症。在麻醉前访视期间,需要做到仔细地询问病史,全面的麻醉体格检查及必要的实验室检查或专科会诊,完成充分告知任务,从而提高小儿眼科麻醉安全性。
The challenge of pediatric ophthalmic anesthesia is how to balance the safety and efficiency of ophthalmic surgery. Careful pre-operative evaluation and preparation can effectively reduce potential complications during pediatric ophthalmic anesthesia. During the preoperative assessment, it’s necessary to gather information, perform a full physical examination, take required laboratory tests or consultations with specialties, and deliver appropriate information and consent to increase the safety of pediatric ophthalmic anesthesia
目的:通过中山大学中山眼科中心眼科互联网护理咨询门诊建设与运营经验,对互联网专科护理服务模式及效果进行分析,为眼科互联网+护理服务提供参考。方法:依托医院优质资源,成立眼科互联网护理咨询门诊;根据眼科患者的实际需求,制订互联网护理咨询门诊服务制度、工作流程,对出诊专家进行资质审核、培训与考核。收集、统计眼科互联网护理咨询门诊接诊量、患者满意度、首诊问题分类及护士职业获益感等数据。结果:2021年6月26日至12月31日累计接诊患者1266人次;接诊时间主要在08:00—22:00;咨询次数最多的问题种类是眼健康知识科普;患者对眼科互联网护理咨询门诊的总体满意度为99.52%;眼科互联网护理咨询门诊运营后,眼科护士职业获益感总分及各维度得分均有提高,差异均有统计学意义(均P<0.05)。结论:眼科互联网护理咨询门诊服务可有效满足各类人群的眼健康护理需求,患者满意度高,亦可提高眼科护理人员的职业获益感,值得进一步推广
Objective: Based on the construction and practice of Internet nurse-led clinic in our hospital, the model and effect of Internet specialized nursing service were analyzed to provide reference for Internet-based nursing service in ophthalmology. Methods: Relying on the hospital’s high-quality resources, the Internet nurse-led clinic in ophthalmology was established. According to the actual needs of ophthalmology patients, the Internet nursing consultation outpatient service system and workflow were formulated, experts’ qualification were reviewed, and training and assessment for the experts were conducted. The data on the number of outpatient visits, patients’ satisfaction, advisory questions and nurses’ perceived professional benefits were collected and analyzed. Results: From 26th June to 31st December 2021, the number of clinical reception was 1 266. The visit time is mainly between 08:00 to 22:00, the most frequently asked questions were eye health knowledge, the overall patients’ satisfaction was 99.52%, and the total score and the score of all dimensions of nurses’ perceived professional benefits were improved, with statistical significance (all P<0.05). Conclusion: Ophthalmic internet nurse-led consultation clinic can effectively meet the health care needs of all kinds of people, with high patient satisfaction, and can also improve nurses’ perceived professional benefits, which is worthy of further promotion.
目的:了解思政元素融入临床医学八年制眼科学整合课程的学生满意度与教学效果。方法:对中山大学临床医学八年制的五年级医学生进行匿名纸质版问卷调查,评估学生对思政教育的满意度与教育效果。对问卷结果进行描述性统计分析,计数资料以频率与百分比的形式表示。结果:共回收有效问卷90份,91%的学生认同在课程中融入思政元素,93%的学生认为思政与课程的融合度为好或者非常好,82%的学生认为思政课程对激发眼科学兴趣有帮助。思政题目的总体正确率高达92.9%,正确率从高到低依次为:医患沟通(100%)、眼科历史(98.9%)、政策要点(83.3%)、研究前沿(82.2%)。结论:临床医学八年制学生对眼科学整合课程的思政元素融合的满意度高。思政教育获得良好效果,学生对思政知识的掌握度高。后续课程在保持学生对医患沟通以及学科历史兴趣的同时,还应加强对于研究前沿和政策要点的教育。
Objective: To investigate the student satisfaction and educational effect of integrating ideological and political elements into the eight-year clinical medicine ophthalmology integrated curriculum. Methods: An anonymous paper questionnaire survey was administered to fifth-grade clinical medicine students ofthe eight-year clinical medicine program of Sun Yat-sen University to assess students’ satisfaction with ideological and political education as well as the educational effectiveness. Descriptive statistical analysis was performed on thequestionnaire results, and the qualitative data were represented by frequency and percentage. Results: Ninety valid surveys were collected. 91% of the students agreed with the incorporation of ideological and political elements into the curriculum, 93% of the students thought the incorporation of ideological and political elements into the curriculum was good or very good, and 82% of the students believed that the ideological and political curricula helped arouse their interest in ophthalmology. The overall correct rate for ideological and political questions reached 92.9%, with the following correct rates from most accurate to least accurate: doctor-patient communication (100%), history of ophthalmology (98.9%), policy (83.3%), and research frontier (82.2%). Conclusion: Eight-year clinical medicine students are highly satisfied with the integration of intellectual and political components into the ophthalmology integration curriculum. Ideological and political education has yielded favorable results, with students demonstrating a thorough grasp of ideological and political facts. Future curricula should expand students’ awareness of research frontiers and policy issues while maintaining their interest in doctor-patient communication and the history of ophthalmology.
全科医生对个人及其家庭所提供的基本、连续性、综合性及整体性的医疗服务。全科医生要求具备更为“广博”的知识结构。那么如何在有限的培训期内更好地完成亚专科的培训并获得社区卫生服务中切实需要的临床技能及知识储备,是目前亟待解决的问题。基于社区卫生服务的基本特点,全科医生规范化培训中培训眼科学的相关内容,强调重视眼与全身疾病、眼科急症及全身药物的眼部损害等,有助于全科医生迅速、准确地发现和判断在社区医疗机构就诊的各类眼部急危重症患者,及时向专业眼科机构转诊,避免延误诊治,为后续专业的眼科治疗赢得宝贵时间,从而使全科医生更好地胜任社区居民健康“守门人”的工作。
General practitioners provide basic, continuous, comprehensive and holistic medical services to individuals and their families, which requires them to have a more “extensive” knowledge structure. However, within the limited training period, how to successfully complete subspecialty training and acquire the clinical skills and knowledge needed in community health services is an urgent problem which need to be explored. Based on the basic characteristics of community health service, the ophthalmology training of the standardized training of general practitioners, focus on the eye and systemic diseases, ophthalmic emergency and side-effect of systemic drug on eye would help general practitioners rapidly and accurately find and diagnose all kinds of serious emergent ophthalmologic diseases, and then refer to professional eye institutions in time, which avoid delay in diagnosis and treatment and save more valuable time for subsequent professional treatment. It would make the general practitioners become good healthy gatekeepers for the local community residents.
随着亚太眼科学会(Asia-Pacific Academy of Ophthalmology,APAO)永久会址和秘书处落户在中山大学中山眼科中心珠江新城院区眼科学国家重点实验室大楼(中国广州),在大数据时代背景下,建设先进的亚太级眼科学科学数据中心已成为亚太眼科创新战略发展的需要。通过建设科学数据中心,一方面缓解眼科学国家重点实验室项目科研数据增长压力,一方面为眼科大数据与人工智能等新兴科研项目开展提供保障平台,为亚太眼科学科建设打下坚实的数据基础。科学数据中心设计建设遵照国家《科学数据管理办法》实施,以加强和规范科学数据管理,保障科学数据安全,提高开放共享水平,更好支撑国家科技创新、经济社会发展和符合国家信息安全建设方针。
As the Asia-Pacific Academy of Ophthalmology (APAO) secretariat permanently located in the building of State Key Laboratory of Ophthalmology (Guangzhou, China). In big data time, it’s a problem for the development of the Asia-pacific ophthalmology innovation strategy to construct the Asia-pacific science data center of ophthalmology. Through the construction of science data center, alleviate the pressure of the state key laboratory of ophthalmology data growth, for big data and new research fields such as artificial intelligence to provide guarantee, provided a basis for the Asia-pacific eye subject construction. Science data center construction in accordance with national “scientific data management method”, to strengthen and standardize the scientific data management, to ensure the safety of scientific data, improve the level of open sharing, better support scientific and technological innovation, economic and social development and national security as a policy.
